Anorexia nervosa is a common condition of the gastrointestinal tract in children with a long period of loss of appetite, aversion to eating, or even refusal to eat, without other diseases. It is caused by the failure of the spleen to transport the stomach, and generally has a good prognosis. However, if anorexia is prolonged, the intake is imbalanced, the absorption of nutrients is impaired, the qi and blood are not full, often secondary to noma, anemia, malnutrition, rickets, resulting in growth retardation of the child; at the same time, because of the deficiency of qi and blood, the resistance to disease is reduced, and the child is susceptible to recurrent colds, coughs, bronchitis and other common diseases of the respiratory tract. Anorexia is often not an independent disease, but a common complaint in pediatrics. The “Straight Guide to Pediatric Medicine” says: “The spleen and stomach are not in harmony and cannot eat milk, resulting in muscle thinness, also because the spleen and stomach are still weak after a major illness or vomiting and diarrhea, and cannot transmit and transform grain gas”. Therefore, we emphasize that we should first take a careful medical history, make a good physical examination and the necessary laboratory tests. It is important to distinguish whether it is caused by systemic or digestive diseases, whether it is affected by drugs, and whether there is a deficiency of trace elements or endocrine elements. It is also necessary to investigate the child’s family, nursery and school environment, the presence of adverse mental stimuli and poor dietary and hygienic habits. Only then can we determine the cause and improve the effectiveness of treatment. Most of the occurrence of anorexia nervosa in children is due to improper feeding, excessive or excessive meal addiction to sorghum and thick flavors and cold drinks, resulting in digestive function failure. “The spleen and the stomach are the basis of the latter part of the body, which transports water and grain and distributes essence, and is the source of biochemistry of qi and blood. The physiological characteristics of children are “delicate internal organs, spleen is often insufficient”, if the innate endowment is insufficient, the spleen and stomach gas is weak, coupled with the inappropriate regulation of the latter day, the diet is out of order, thick flavors hurt the middle, and eventually lead to the spleen and stomach transport and transformation of the failure, so there is a lack of appetite and anorexia. As stated in the Nei Jing, “The stomach is injured when the diet is doubled.” Due to parents’ blind pursuit of high-protein and high-calorie feeding and the change of children’s diet structure nowadays, it has been suggested that the cause of anorexia nervosa in children is mainly fatty, sweet, thick and greasy overfeeding, and the main type of evidence is wet food and stagnant food; according to statistics, children of this type account for about 80% of anorexic children. Clinical manifestations of anorexic children with wet food and stagnation: aversion to eating, distention and fullness of the abdomen and stomach, bad breath and sweating, restlessness, dry stools or constipation, red tongue with greasy or yellowish coating, sunken and strong pulse, purple-red or deep purple fingerprints, and Feng Guan. There is mucus from acupuncture at the four suture points. The diagnosis is that the vegetative spleen is deficient and heavy with dampness, improper feeding, stagnation of damp food and loss of spleen health. Treatment: Eliminate stagnation, transport the spleen and dry dampness. Radicals: To add and subtract Baohe Pill and Pingguisan. Commonly used herbs: Atractylodes macrocephala, Chuan Park, Chen Pi, Sempervivum, Poria, Lycopodium, Forsythia, Neijin, Hawthorn, etc. Combined with acupuncture at the “four sutures” point, it is especially suitable for the type of dampness and food stagnation. The clinical manifestations of the spleen and stomach Qi deficiency type are lack of desire to eat, less florid complexion, weakness and laziness in speech, light tongue with thin coating, thin and soft pulse, or light red fingerprints, less food and more stools, thin and soft stools, or with indigestible food. The spleen and stomach are deficient in qi and have no power to transport and transform. Treatment: Benefit Qi, strengthen the spleen, and help transport and awaken the stomach. Remedy: Isogong San or Xiang Sha Liu Jun Wan with flavor. The clinical manifestations of Stomach-Yin deficiency type are less food and more drinks, dry and difficult stools, withered face, thin body, skin not moistened, red tongue with little moss, or flaking moss, fine and small pulse, or red fingerprints. Stomach Yin deficiency, incompetence in receiving and receiving. Treatment: Nourish the stomach and generate fluid by transforming Yin with acid and sweetness. Remedy: Nourish the stomach and increase loss. Clinical manifestations: loss of appetite, spontaneous sweating and night sweating, cool sweating limbs, banging face with less floridness, susceptible to external evil, soft abdomen and bowel movement, or dewy eyes during sleep, light red tongue, thin moist moss, fine and harmonious pulse, or light red fingerprints, and wind shutdown, are due to disharmony of Ying and Wei, which affects dysregulation of Qi flow of the spleen and stomach. This kind of children mostly suffer from recurrent colds and improper feeding, which damage the spleen and stomach, resulting in more sweating and easier sensation and loss of appetite. Treatment: Harmonize Ying and Wei to promote awakening of gastric energy. Remedy: Anorexia nervosa. It is derived from Gui Zhi Tang with addition of flavor, and is composed of: Gui Zhi, Bai Shao, Roasted Licorice, Chen Pi, Fructus, Fried Grain Bud, Ginger and Jujube. For sweating and drenching susceptibility, add Fangfeng, Astragalus, glutinous rice hard, Atractylodes; for abdominal pain and soft stool, add fried lentils and jiao hawthorn curd; for red tongue with little coating or epistaxis, add Dendrobium Chuan and fried lotus root; for dry stool, add smallpox powder and fire hemp seed. Clinical manifestations of liver and stomach disharmony: not thinking about food and drink, belching and nausea, irritability, restlessness at night, blue and yellow face, facial or mountain root veins showing, reddish tongue, thin yellow moss, string pulse or blue and purple stagnant fingerprints. The liver and stomach are not in harmony, thoughts hurt the spleen, the qi reverses, and the nasal transportation is abnormal. Treatment: Diversify the liver and regulate qi, strengthen the spleen and harmonize the stomach. Direction: Diverging the liver and Qi, strengthening the spleen and harmonizing the stomach.
