Those who have urine in the bladder for 6h to 8h after delivery and cannot discharge it by themselves are called postpartum urinary retention. It is one of the common complications of obstetrics, often affecting uterine contraction, leading to increased vaginal bleeding, increasing pain to the mother, and also an important factor causing postpartum urinary tract infection: I. Pathogenesis: 1. The mother is not used to bed urination and leads to urinary retention; 2. The labor is long, the bladder and urethra are pressed by the fetal first dew for too long, leading to bladder and urethra mucous membrane congestion and edema, tension becomes low and urinary retention occurs; 3. The abdominal wall is relaxed due to long lasting dilatation during pregnancy, and the abdominal pressure decreases, resulting in the inability to urinate; 4. Postpartum perineal lateral incision or perineal tear causes painful vulvar trauma, which disrupts the nerve function of the innervated bladder and reflexively causes spasm of the bladder sphincter and postpartum urinary retention; 5. Sedative drugs, such as magnesium sulfate, scopolamine and other drugs used in gestational hypertension syndrome, can reduce bladder tension and cause urinary retention. The patient should be able to listen to the sound of running water and use the conditioned reflex to ease the inhibition of urination, so as to make the patient feel the urge to urinate and prompt urination. 2.Hot compress method Place a hot towel or hot water bag on the bladder area of the patient’s lower abdomen and use heat to make the relaxed abdominal muscles contract and increase abdominal pressure to promote urination. 3.Massage method Place your hand on the bladder bulge of the patient’s lower abdomen and massage gently to the left and right for 10 to 20 times, then use the palm of your hand to press downward from the bottom of the patient’s bladder to reduce residual bladder urine; 3.Acupuncture treatment Acupuncture points can be taken on the abdomen and extremities, such as Zhongji, Guanyuan, Qihai, Sanyinjiao, etc. Qihai, Sanyinjiao, etc., can be connected to electric needles, stay for 20-30 minutes, usually 1-2 times of treatment to urinate. Conclusion: Acupuncture therapy has significant efficacy and no side effects, and is worthy of clinical promotion.
