How to add or subtract Shengqi Tang to treat Qi deficiency cardiovascular disease?

The original formula of Shengchi Tang was derived from the “Medical Genjing Ginseng and Xi Lu” and was used to treat the evidence of atmospheric subsidence in the chest with good efficacy [1]. The clinical manifestations and pathogenesis of qi deficiency cardiovascular disease are similar to those of atmospheric subsidence, so we have developed a new formula for the treatment of qi deficiency cardiovascular disease by adding and subtracting Shengqi Tang, based on the effective experience of Shengqi Tang, which has achieved very good results in clinical practice. 1. The composition and explanation of the formula of Shengchi Tang are as follows: 30-60 g of raw Astragalus, 15-30 g of Radix Codonopsis, 15 g of fried Atractylodes, 20 g of Radix et Rhizoma Atractylodis, 10 g of Pericarpium Citri Reticulata, 6 g of Radix Bupleurum, 10 g of Zhi Mu, 5 g of roasted licorice. The dosage is determined according to the degree of Qi deficiency and the quality of Astragalus, usually 30-40 g of extra-grade and first grade products, 50-60 g of second grade products. The dosage is determined by the degree of Qi deficiency and the quality of Astragalus, usually 30-40g for the first grade and 50-60g for the second grade; other grades of Astragalus are mostly of poor quality and we do not advocate using it for Qi deficiency heart disease. Radix Codonopsis Pilosulae tonifies the heart and spleen, Atractylodes Macrocephala strengthens the spleen, tonifies the qi, and tonifies the backbone of the body, while Radix Angelicae Sinensis tonifies the kidney qi and consolidates the backbone of the body. Chen Pi is used to regulate Qi, resolve phlegm and strengthen the spleen, so as to tonify without stagnation; Chai Hu elevates the Qi and relaxes Shao Yang; Zhi Mu cools and moistens and restrains the large team of Qi tonics that tend to generate internal heat. Roasted licorice benefits the qi and the middle, and harmonizes all medicines. 2, common concurrent evidence of common addition and subtraction Qi deficiency cardiovascular disease may appear a variety of concurrent evidence, the symptoms of deficiency mixed, must be based on the evidence of concurrent evidence and physical condition, flexible addition and subtraction, in order to achieve the desired effect. In case of kidney deficiency and heart deficiency, add Cornu Cervi Pantotrichum and Wu Wei Zi to strengthen the kidney and astringent heart. For those who also see yin deficiency, add Mai Dong and Yu Zhu to nourish yin and nourish the heart. For obvious Qi stagnation, reduce the dosage of Qi tonics and add Radix Aromaticus and Psidium Guajava. For obvious Qi subluxation, add small amounts of Radix Platycodon and Radix et Rhizoma to strengthen the lifting power. For stagnation of blood stasis, add Dan Shen, Yu Jin and Chuan Xiong to invigorate blood circulation and remove blood stasis. If the heart yang is not revitalized, add Gui Zhi and Allium, and remove Zhi Mu to warm yang and disperse nodules. For those with internal heat toxicity, add Scutellaria baicalensis, Panax quinquefolium, and Jin Yin Hua to clear heat and detoxify the heart and protect the myocardium. 3. Case presentation Cardiac neurosis Ning, male, 50 years old. The patient had recurrent episodes of chest tightness and shortness of breath from 4 months ago, which could be relieved for a while by taking quick-acting heart pills. He was diagnosed with cardiac neurosis and was given oral treatment with glutamate, Valium tablets, and TCM, but his symptoms remained the same. On examination, the tongue was light red with white coating and the pulse was thin and sunken and weak. The evidence belongs to deficiency of heart qi and qi trapping, no obvious qi stagnation performance, if presumptuous use of rational qi diversion, will further deplete the heart qi, then the symptoms aggravate. The treatment should be based on tonifying the Qi of the heart and lung, together with lifting and opening the chest, by adding and subtracting Shengqi Tang with 5g each of Radix Platycodon grandiflorus and Shengma and 15g of Psidium guajava, and the symptoms were relieved for more than a month. Note: Due to the increased pace and pressure of work in modern society, if the cardiac Qi is exceeded for a long period of time to mediate the load, depleting the heart’s Qi, it can also lead to this disease. We should not assume that the dysfunction is due to Liver-Qi stagnation and use Qi-regulating and Liver-suppressing products. Arrhythmia of coronary heart disease Xie, male, 63 years old. He has been suffering from coronary artery disease for 5 years and paroxysmal atrial fibrillation for 2 years. 2 months ago, his symptoms worsened with frequent palpitations, prolonged duration, shortness of breath and weakness, which were aggravated by light physical activity, and was treated with oral amiodarone for 6 weeks, but his symptoms did not improve. His disease is long-standing heart qi is greatly deficient, the heart veins are not nourished, the heart disease gradually involves the kidney qi over time, and the kidney’s ability to absorb qi has decreased. In this disease, we should strengthen the consolidation of kidney qi while greatly nourishing the heart qi, and add 15g of Cornu Cervi Pantotrichum, 20g of Mai Dong, 15g of Wu Wei Zi, 20g of Gua Pou, 25g of Danshen, 30g of raw keel, 30g of raw oyster, and 15g of roasted licorice to treat 1 month later, the strength increased and turned into occasional atrial fibrillation, and then consolidate the treatment for 2 months, atrial fibrillation did not recur, and now intermittent amiodarone can Sinus rhythm was maintained. The patient with atrial fibrillation has a deficiency of heart and kidney qi, but yin deficiency is often not obvious. The dosage of yin tonics should not be too heavy to prevent stagnation of qi and add to the evidence of change. Viral myocarditis Xia, female, 23 years old. The patient had diarrhea and abdominal pain paroxysms with sore throat for 2 days 3 months ago, the former symptoms were relieved after 5 days, gradually developed weakness and shortness of breath, not relieved after rest, went to the local hospital and found that the myocardial enzyme profile was more than 2 times elevated, diagnosed as viral myocarditis, given a variety of myocardial nutrients, no significant relief of symptoms, at the time of the visit had been suspended, the tongue was red and red with white coating, the pulse was thin and sunken taking weakness. This is due to the entry of external evil toxins into the heart and a large deficiency of heart qi. He was treated with a combination of Qi tonification, heat clearing and detoxification, and was given an addition and subtraction of Shengchi Tang with 15g of Scutellaria baicalensis, 20g of Panax quinquefolium, 20g of Gymnema sylvestre, and 5g of Shengma, and recovered completely in 3 months with the addition and subtraction of treatment. The effect of purely clearing away heat and detoxifying the toxin is poor, and the abuse of blood-breaking drugs will injure the vital energy. The raw Astragalus, Panax quinquefolium, and Ginkgo biloba in the formula have a number of experimental studies to confirm the clear antiviral and myocardial protective effects [2]. Clinical selection of the flavors that are based on experimental studies is also a way to improve the efficacy. Chronic heart failure Bai, male, 12 years old. The patient developed weakness and palpitations after a cold six months ago, and gradually worsened, and was seen at several provincial hospitals, where he was found to have a number of substantially elevated cardiac enzyme profiles, an enlarged heart, and decreased cardiac function, and was diagnosed with viral myocarditis and heart failure, and was treated orally with cardiotonic and vasodilatory western drugs and a variety of myocardial nutrients for nearly six months, with some improvement in cardiac function indicators and an enlarged heart, and still had shortness of breath and weakness at the time of the visit, and walking 30 He still had shortness of breath and shortness of breath at the time of consultation, and could not go to school because of shortness of breath after walking 30 meters. He was treated with addition and subtraction of Shengzhi Tang with Cornus officinalis 20g, Schisandra chinensis 10g, Psidium guajava 15g, Allium officinale 10g, Cinnamomum officinale 10g, Medlar 15g, Radix et Rhizoma pinnatifida 10g, Salviae Miltiorrhizae 15g, and removed Zhi Mu, and his heart function gradually returned to normal, and the maximum transverse diameter of each chamber of the heart returned to normal high limit. The patient’s physical condition has gradually improved and he has returned to school. In this case, although it is a child, the condition is serious and dominated by positive deficiency, which is caused by deficiency of heart qi and kidney, and kidney does not receive qi, while the evil is not obvious, so no heat-clearing and detoxifying drugs were used. The strong pulse is due to the effect of cardiac and vasodilator western drugs, not the kidney Qi to be supplemented to turn the image of the real, identification of evidence must exclude interference, identify the essence of the symptoms. 4.Summary Adding and subtracting Sheng shen Tang is the basic core formula for treating Qi deficiency cardiovascular disease that we have concluded in our long-term clinical work, with reliable efficacy and wide range of application. As the Qi deficiency is obvious in this type of disease, but the Qi trapping is not serious, so the number of flavors and doses of Qi tonics are increased, and the number of flavors of elevating drugs is reduced, which is in line with current clinical practice and is a development of the original formula of Zhang Xichun’s Elevating Trapping Tang, which is worth promoting. As this formula is mainly for tonifying, it should be noted that it should not be applied to those who are suffering from evil heat and stagnation, hyperactivity of liver and Yang, and yin deficiency and fire.
