Foam sclerotherapy to help young women with capillary dilation

Abstract: This article describes a patient with early varicose veins presenting as dilated capillaries who came to our department because she was unable to wear a dress in summer, which affected her aesthetic appearance. The ultrasound clarified that the main trunk of the saphenous vein was not dilated, and foam sclerotherapy injection was used to treat the local capillary dilation. The procedure was performed by local anesthesia without incision, and the patient could be discharged on the same day, with good postoperative recovery and no obvious complaints of discomfort at follow-up. Initial interview 2021 In November, a 26-year-old female patient came to the department with “capillary dilation for 5 years”. The patient was a salesperson in a shopping mall. She developed capillary dilation 5 years ago without any obvious cause, and had a significant soreness and swelling in her lower limbs after sitting and walking for a long time. The patient’s history showed that the patient was physically fit in the past, and his father had a history of varicose veins, which had been treated surgically. The patient had a history of varicose veins in the right lower leg, which affected the aesthetics, especially in summer, and prevented him from wearing skirts and shorts. Physical examination: local capillary dilation was seen in the right lower limb, with normal skin color, slightly high skin temperature, palpable arterial pulsation, and fair sensorimotor function. After the patient was admitted to the hospital, relevant examinations were completed and the condition was evaluated. Venous ultrasound of the lower extremities suggested that the right saphenous vein showed no significant abnormality. The patient’s current varicose veins were classified as CEAP grade C1, representing capillary dilation, and physical therapy such as compression stockings was recommended under normal circumstances. However, the patient was a young woman with high aesthetic requirements, so the patient was recommended to use local injection of sclerosing agent, which is a less invasive method without incision and can be discharged on the same day. The patient had a smooth surgical operation and recovered well after surgery. The operation was performed under local anesthesia, and the patient could be discharged after the operation, observed for 2 hours, and discharged by appointment, with a review 3 days after discharge and replacement of compression stockings. Three days after the operation, the patient returned for review, and it was seen that the skin temperature and skin color were normal, and local hard nodes appeared at the vascular injection drug, the patient was given a change of incision, the elastic bandage was removed, and the medical compression stocking was replaced to continue to promote venous reflux. 1 month later, the patient was reviewed, and it was seen that the capillary dilatation disappeared significantly, and the hard nodes disappeared, and the effect was satisfactory. Fourth, precautions patients after surgery capillary dilation disappeared, for patients feel very happy, but also need to pay attention to the following matters: 1, foam sclerosing agent injection often produces local hard knots in the blood vessels, is a normal phenomenon, the correct wear pressure stockings, will accelerate the absorption of hard knots and recovery. 2, capillary dilation patients should avoid standing and sitting for a long time, but also appropriate to increase physical exercise to enhance the function of calf muscles, thereby promoting venous reflux. V. Personal insight Capillary dilation belongs to mild varicose veins, often patients have no obvious vascular protrusion, which is shown as spider-like capillaries or reticulated capillaries. The foam sclerotherapy injection alone is only suitable for patients with mild varicose veins, which have the risk of recurrence. When choosing the modality clinically, it is often combined with surgical modalities such as stripping, laser and radiofrequency to reduce the recurrence rate. Under normal circumstances, patients are advised to avoid sitting and standing for long periods of time, to elevate their legs at rest, and to wear medical compression stockings properly to slow down further development.
