How to choose Chinese patent medicine for patients with coronary heart disease

How to choose Chinese patent medicine for coronary heart disease patients Among many cardiovascular diseases, coronary heart disease has become one of the most serious “killers”. Among the medical treatments for coronary heart disease, traditional Chinese medicine has been widely recognized for its efficacy in improving the clinical symptoms of coronary heart disease, and has advantages that cannot be replaced by other treatments. At present, Chinese patent medicines used in the treatment of coronary heart disease can be divided into three main categories, namely, activating blood circulation and resolving blood stasis, aromatic and warming, and fuzheng and nervine, among which the most widely used Chinese patent medicines for coronary heart disease are those that activate blood circulation and resolve blood stasis. There are many traditional Chinese medicines with the function of activating blood circulation and resolving blood stasis. From modern pharmacological studies, it seems that their mechanisms of action are similar, such as regulation of blood lipids, anti-atherosclerosis, anti-platelet aggregation, protection of vascular endothelium, improvement of microcirculation, and antioxidation. However, the medicinal properties (cold, hot, warm, cool, flavor, normalization, elevation, floating and sinking, etc.) contained in the different appearance of each Chinese medicine have their own characteristics, therefore, the efficacy characteristics and indications shown in clinical practice are different. Coupled with the combination of multiple drugs in proprietary Chinese medicines, it is even more important to consider their comprehensive medicinal properties and use them accurately. At present, there are many proprietary Chinese medicines for coronary heart disease, angina pectoris and myocardial ischemia in the market, but the people and many western doctors often look at the “main treatment” of Chinese medicines because they do not understand the “function” of Chinese medicines, resulting in inaccurate identification and use of medicines. Specifically, for example, drugs containing ginseng, Chinese herbs, cinnamon and toadstool are often hot, and their main effect is to “strengthen the heart”, so they are suitable for patients with “cold signs” and heart failure symptoms. At the same time, because of the presence of toadstool, long-term use requires attention to the occurrence of cardiac arrhythmia, so it is best to consult a Chinese medicine practitioner before taking. For example, drugs containing worms such as leeches, dirong, centipede, etc. are classified as “blood-breaking” drugs in Chinese medicine because they contain enzymes that dissolve blood clots, and their specialty is antithrombotic and thrombolytic treatment for patients with heart and brain infarction. However, it should be noted that these worm medicines contain a large amount of allogeneic proteins, which may cause allergic reactions in some patients, and the changes in liver enzymes should be noted in long-term use. In addition, even for the same botanicals, Chuan Dou and Danshen have obvious differences in clinical indications, which is often the most common misconception that people and even doctors can easily fall into. Chuan dome “pungent and warm, up the head, down the blood sea, in the open depression”; Dan Shen “slightly cold, descending and blood flow, from heat and stagnation is appropriate”. Therefore, for those who have myocardial ischemia with insufficient blood supply to the brain and unheated extremities, it is more suitable to use Chuan Dou rather than Danshen, and for those who have strong body, hot blood, and troubled heart, Danshen can be used to cool the blood and remove the trouble. Modern pharmacological research also confirmed that Chuan Dou and Dan Shen can improve cerebral microcirculation, but Chuan Dou can significantly increase the effective cerebral blood volume, while Dan Shen does not increase cerebral blood volume, and also slightly reduced. This is also the reason for not infusing Salvia officinalis in the treatment of cervical vertigo (cerebral blood supply deficiency caused by cervical spondylosis), but infusing Chuan Dome. At present, the main types of Chinese patent medicines for treating coronary heart disease are compound danshin tablets, quick-acting heart pills and Tongxinluo capsules. It can be used not only during acute attacks of angina pectoris, but also for prevention and treatment before and after the onset of symptoms. After thirty years of extensive clinical use, the drug has won the trust of the majority of doctors and patients. The combination of Chuan Dome and Bing Pian is pungent and warm, while Bing Pian is slightly cold, with moderate medicinal properties, and its mechanism of action is more focused on improving blood supply, which is more suitable for long-term application for people with myocardial ischemia and cerebral insufficiency, such as dizziness, insomnia and amnesia. Moreover, Chuan Dou is respected as the “qi of blood” among the blood-stasis activating drugs, activating blood and moving qi, so it is more suitable for patients with “qi stagnation”. Although Panax ginseng is warm, it accounts for a very small proportion of the prescription. Therefore, the overall medicinal properties of Danshin tablets are cooler, so people with hot constitution will get better results when using them. Therefore, in the process of using Chinese patent medicines to prevent and treat coronary heart disease, it is necessary to take into account the individual characteristics and disease conditions of different patients and treat them according to the evidence in order to better utilize the advantages of Chinese patent medicines in the prevention and treatment of coronary heart disease.
