PG-SGA: Operation Manual for Nutritional Status Assessment of Tumor Patients Foreword Shi Hanping, Department of General Surgery, Aviation General Hospital of China Medical University On March 17, 2012, the Professional Committee of Tumor Nutrition and Supportive Treatment of China Anti-Cancer Association was officially established in Guangzhou Hegun Building. At the beginning of the establishment of the society, we planned to conduct a large-scale survey on the nutritional status of tumor patients nationwide, hoping to answer two questions with bulk data: first, the incidence of malnutrition in different tumors; second, the incidence of malnutrition in different stages of the same tumor. Because up to now, there is not a large scale survey data of nutritional status of oncology patients all over the world, and the available data vary greatly, the prevalence of malnutrition in oncology patients is 19-71%. This brings great confusion to oncology clinical workers and seriously affects the reasonable treatment of tumor patients, which in turn affects the quality of life and shortens the survival time of tumor patients. The work is good, but the tool is good. There are many tools that can be used to evaluate the nutritional status of tumor patients, such as MUST, SGA, BCA, MSTC, PNI, PG-SGA, etc. Each tool has its own characteristics and drawbacks, and none of them can be called perfect. The PG-SGA (Patient-Generated Subjective Global Assessment) is the most ideal and widely used assessment tool in the subjective global assessment (SGA). The PG-SGA (Patient-Generated Subjective Global Assessment) was developed based on the Subjective Global Assessment (SGA). It was first proposed by Ottery FD in 1994 as a nutritional status assessment method specifically designed for oncology patients. Clinical studies have shown that PG-SGA is a valid tool for assessing the nutritional status of oncology patients, and therefore it has been highly recommended and widely used by the American Dietetic Association (ADA) and other organizations. This is the reason why our society decided to adopt PG-SGA to conduct a large-scale survey on the nutritional status of oncology patients. When our society was ready to adopt PG-SGA to evaluate the nutritional status of oncology patients with confidence and hesitation, we encountered a roadblock: we did not know how to operate the PG-SGA and how to score? I personally humbly and sincerely consulted dozens of famous clinical nutrition experts in China, including those who had already used PG-SGA to investigate patients and published papers, but none of them gave a convincing answer, and there were different opinions and confusion. On October 15, 2012, I wrote my first letter to Emily Jeffery for advice, and since then, I have consulted the authors of PG-SGA-related papers in the United States, the United Kingdom, Australia, Japan, and other countries. Among them, Emily Among them, Emily Jeffery and Dana Middleton not only explained the key points and scoring criteria of the PG-SGA, but also provided me with the important information that there are DVDs of the PG-SGA for sale in the U.S., and gave me the website address. I was so delighted that I immediately went online to buy the DVD, but at that time the website did not sell PG-SGA DVDs to mainland China, so I had no choice but to ask a friend who worked in the U.S. to buy it for me. After receiving the PG-SGA DVD, I watched and studied it more than 10 times and started to apply it in my own department. Through study and practice, we completely mastered the standardized operation and scoring criteria of PG-SGA. Later, our society made SOP slides of standardized PG-SGA operation and sent them to members all over the country for promotion and publicity. Considering that the impact of the slides was small and the beneficiary was narrow, we decided to write a book dedicated to the standardized operation and scoring criteria of PG-SGA. We received strong support and enthusiastic help from People’s Health Publishing House for this idea, which led to this book. This manual is based on the DVD of PG-SGA operation recorded by the Academy of Nutrition and Dietetics (AND), combined with our own practical work experience, and some of the pictures are from the DVD of AND. We hope that the American AND will contact the People’s Health Publishing House and the author in time after seeing this book, and we will pay the relevant fees according to the international copyright law. This book is the result of the joint efforts of medical and nursing staff, tumor patients, the collective wisdom of all the experts of the Professional Committee of Tumor Nutrition and Supportive Treatment of China Anti-Cancer Association, and the selfless dedication of my dear, lovely and respectable patient friends. It is expected that this book can standardize the practical operation of PG-SGA and promote the clinical application of PG-SGA, thus benefiting the majority of tumor patients in China.
