Recently, there are a lot of questions from patients: Doctor, can advanced colorectal cancer be cured? Can it only be maintained by medication? Is it possible to live longer? Whenever I hear these questions, I feel helpless and sad at the same time. It seems that most patients do not know much about colorectal cancer. Here, I will talk about the treatment of advanced colorectal cancer. First of all, what is advanced colorectal cancer? Many people think that having metastasis is advanced, and some people think that not being cut off by surgery is advanced. In fact, these views are incorrect and one-sided. First of all, even if a patient has metastasis, whether it is lymph node metastasis or surrounding organ invasion, if the metastasis is within the scope of surgical clearance, this situation is not advanced, or at best, it can only be considered as locally progressive stage. Secondly, even if there are metastases outside the scope of colorectal surgery, if they can be cured by other surgical or therapeutic means, they cannot be considered advanced in the pure sense. According to the latest version of international staging, stage IV (that is, the most advanced stage) colorectal cancer is divided into stage IVa and stage IVb, while stage IVa still has a greater hope of cure. The most common distant metastases of colorectal cancer are liver metastases and lung metastases, and most of the people usually refer to the occurrence of liver metastases or lung metastases when they talk about advanced stage. People usually think that the occurrence of liver metastasis or lung metastasis means the chance of surgery is lost, and it means going back to “waiting”. In the past 10 years, a large number of studies have shown that for patients with liver metastasis or lung metastasis alone, nearly 40%-50% of them can be surgically removed or combined with other means to achieve long-term survival. Although our country started a little late in this area, in recent years, China has made great progress in the treatment of metastatic colorectal cancer, and the treatment effect has reached the international level. In fact, the significance of treatment decision of advanced colorectal cancer is much more than the treatment means. There are various means of treatment for advanced colorectal cancer, and there are sufficient options, such as surgery, chemotherapy, radiofrequency therapy, radiotherapy, interventional therapy, etc. All of them have certain roles to play, so how should the treatment be done in order to make the patients get the most benefit? At present, the most advocated treatment model is the multidisciplinary team model, that is, a specialized, high-level treatment team composed of experts from surgery, chemotherapy, radiotherapy, imaging and pathology, to develop a reasonable treatment plan for each advanced patient’s situation. Finally, the majority of patients must build confidence. Late stage” does not mean “incurable” and “late stage” does not mean “not long to live”, as long as the treatment is regular, late stage colorectal cancer is very hopeful to be cured!
