Hemorrhoids are a common disease, and as the saying goes, “nine out of ten people have hemorrhoids”, and it is relatively common for people to have hemorrhoids. But many people think that hemorrhoids are a small disease that won’t kill you and can be treated or not. The actual hemorrhoids can also be a big problem. In the process of receiving patients, there are often patients who come to the hospital thinking that they have a minor problem – hemorrhoids, but after a detailed examination, it is eventually found to be rectal cancer, and it is often found to be at an advanced stage. The causes of hemorrhoids are complex, and changing bad habits hemorrhoids are still complex, and in general are related to anatomical factors, dietary factors, bowel habits, pregnancy and childbirth, chronic diseases, and occupational age. The first is the anatomical structure of the person. The superior rectal vein and its branches have no venous valves, and when a person is standing or sitting, due to the influence of geocentric attraction, it is easy to cause the venous plexus in the anorectal area to stagnate and expand, and if it is in this constant position for a long time, hemorrhoids will be formed; here, the tissue of the submucosa of the rectum is loose, and the venous plexus is easy to expand and stagnate into hemorrhoids. The next reason is the occupation. The occupations that require long standing and sitting are prone to induce hemorrhoids, such as store attendants, office workers, cab drivers, etc. Lifestyle habits are also an important factor. Some people like to eat spicy, such as often eat chili, pepper, mustard, cumin, wine, etc., can directly stimulate the anorectal mucosa, excessive diet after the stool will feel burning pain, triggering or aggravating hemorrhoids. There is a role; there are people who are used to reading books and newspapers during defecation, and the length of squatting has a significant relationship with the incidence of hemorrhoids. A survey shows that the incidence of hemorrhoids can reach 72.41% for each squatting toilet for more than 10 minutes; 57.88% for those within 10 minutes. Therefore, squatting for a long time is a bad defecation habit and an important cause of hemorrhoids. In addition, the incidence of hemorrhoids is high in people with chronic diarrhea, chronic diarrhea, and chronic constipation. If the blood in the stool repeatedly becomes a major disease hemorrhoids early is a minor disease, the symptoms are not obvious, but if repeatedly bleeding, prolapse, it must be treated, if the hemorrhoids paralysis will delay the disease. In the hemorrhoid patients seen in the clinic, the amount of blood in the stool is a few dozen milliliters, more than a few hundred milliliters, serious jet bleeding, but the patient believes that no pain, not serious, they use some hemorrhoid medicine, the symptoms are light, you think the disease is well. Over time, the patient appears pale, dizzy, general weakness, loss of appetite, accelerated heart rate, swelling, shortness of breath after physical activity, finally causing anemia and even hemorrhagic shock. Some patients also have prolapsed hemorrhoid nuclei in the stool, which gradually increases in size as the condition prolongs and develops. If they are not retracted in time, the patient will experience symptoms such as urgency and anal swelling, and over time, mixed hemorrhoids will become embedded and infected with necrosis. If the treatment is not timely, it is easy to cause the spread of infection and the secondary development of perianal abscess. How should I choose between conservative and surgery? Hemorrhoids have many harmful effects on the human body, but hemorrhoid patients do not have to be overly nervous, only when sex to be able to early treatment and appropriate treatment, can avoid the above serious complications. The early use of conservative therapy for hemorrhoids is effective, such as fumigation, hemorrhoid plugs in the anus, dressing method, oral medication, physical therapy, etc. Short-term efficacy can be, but it is easy to recur. If you believe in conservative treatment, it will delay the disease. If the effect is still not good and the hemorrhoid has significant symptoms such as frequent episodes of bleeding, infection, uncleanliness and itching around the anus, you should consider elective surgery. Hemorrhoid surgery is generally indicated for second, third and fourth stage internal hemorrhoids or ring-mixed combined hemorrhoids. Different surgical methods should be used for different hemorrhoids and cannot be uniform. Currently, the commonly used surgical methods are ligation therapy, minimally invasive surgical method, injection therapy, RPH (ligation therapy) and PPH (suprahemorrhoidal mucosal circumferential stapling), TST (selective suprahemorrhoidal mucosal circumferential stapling), HAL (Doppler-guided hemorrhoidal artery ligation), etc.
