Male, 70 years old, numbness and weakness of the lower extremities for 28 years. 3 months later, he could only walk 300 m with numbness of the fingertips of both hands. CT and MRI were performed and the diagnosis was: cervical spondylosis of the spinal cord (developmental + degenerative cervical spinal stenosis, spinal cord degeneration). MRI lower image of the right spinal cord (long gray strip) with multi-segment compression, cerebrospinal fluid (white) opacification, and spinal cord signal alteration posterior cervical spine Single-opening spinal canal enlargement plasty. The surgical bleeding was 200 ml and the operative time was 87 minutes. The strength and flexibility of the lower limbs improved significantly after surgery. He was discharged from the hospital 5 days after surgery. The rehabilitation unit was recommended for hospitalization and functional exercise under the guidance of a rehabilitation physician. Review MRI preoperatively (left image) Multiple segments of the spinal cord (long gray strip) are compressed, cerebrospinal fluid (white) is not fluent, and the spinal cord is thinning + signal change. After MRI (right image), the spinal cord compression was released and the cerebrospinal fluid was clear, but the spinal cord was thicker than before surgery, and there were still signal changes (white dots in the spinal cord). Surgery can release the compression, but the recovery of neurological function depends on the patient’s own condition. For example, in this patient, who had been ill for 28 years and was not treated early enough, the spinal cord still became thin and degenerated even though there was no more compression after surgery. After surgery, the symptoms generally improve and do not worsen, but the recovery is slow and the improvement rate is low. In 2015, the surgical technique was improved to preserve the muscle-ligament complex, reduce muscle damage, and reduce postoperative pain in the cervical spine. The cervical spine is more flexible and free to move after surgery. Postoperative precautions: 1. Lifelong precautions: pay attention to sitting posture, diligent activity, and strengthen back muscle exercise. 2.Please wear a good collar before getting up after surgery for a total of 2-4 weeks for the purpose of protecting the neck muscles. 3.When lying in bed after surgery, there is no need to wear a collar, and the neck can be moved. 4.After 2-4 weeks, the neck pain disappears, no need to wear the collar, and the neck can move freely.
