Patients with ischemic necrosis of the femoral head cannot bear weight for 12 weeks after hip preservation surgery, and gradually bear weight after 3 to 6 months without pain and obvious discomfort. Therefore, patients should be trained to learn the skills of walking with double and single crutches before surgery. The preoperative training should also focus on the strength of the quadriceps muscle, i.e. quadriceps contraction and ankle dorsiflexion and plantarflexion. That is, straight toe lift and hooked toe lift, preoperative practice four times a day, each time more than 50 times, each action should be held for 10 seconds. There is another focus in the preoperative period is hip abduction training i.e. hip abduction, hip extension and hip rotation training. The correct movements of hip abduction must also be practiced preoperatively. So you want to recover well before and after surgery you must not be afraid to take pains to work hard. Muscle strengthening training: 1. You should do quadriceps contraction exercise and ankle dorsiflexion plantarflexion exercise in the prone position. The muscle strength training should be isometric contraction exercise, mainly training the quadriceps and calf muscles, every time you complete a quadriceps contraction and stretch and ankle dorsiflexion and plantarflexion is one time, each time for 5 seconds. The frequency of activity is 3 times a day, each time 50 times. 2. Gluteus medius training: lying on the side, with the affected limb above, lift the straight leg upward at an angle of 30° for 5 seconds each time. The frequency of activity is 3 times a day, each time 50 times. Joint mobility training: Continuous passive hip movement: move the hip joint by yourself, gradually increase the hip flexion angle as much as you can tolerate, 15-20 times in total. Patients with crutches should get out of bed to perform hip exercises: Standing position with crutches: 1. Straight leg of the whole lower limb should be extended forward for 5 seconds each time, three times a day, 20-50 times each time. 2. Extend the straight leg of the entire lower limb outward for 5 seconds each time, three times a day, 20-50 times each time. 3.Knee joint backward flexion, each time 5 seconds, three times a day, each time 20-50 times. Hip abduction training: 1, hip abduction: lateral lying position. With the affected hip on top, rest the head on the upper arm and flex the limb below to provide support. Slowly raise the affected limb to the side by 45°, keep the hip joint in a straight position, hold this position for 5 seconds each time, slowly lower the affected limb and rest for 2 seconds, this exercise should be attached to the ankle joint with a small sandbag. Do 50 times each time, once in the morning and once in the evening, adhere to 2-3 weeks. 2.Hip extension: prone position. Put a pillow cushion under the hip, and flex the knee joint on the affected hip side by 90°. Raise the lower limb vertically upward and make it leave the bed for 5 seconds, this process should keep the knee joint flexed 90° position, also attach a small sandbag to the ankle joint to increase the load. Do 50 times each time, once in the morning and once in the evening, and insist on 2-3 weeks. 3.Hip rotation: prone position. Put a pillow cushion under the hip and bend the knee joint on the affected hip side by 90°. The hip joint do rotational movements: that is, the ankle joint slowly in a semicircular arc, from one side to the other, the arc of activity as large as possible. This exercise should also be done with a small sandbag attached to the ankle joint. Do this 50 times each time, once in the morning and once in the evening, and stick to it for 2-3 weeks.
