With the further development of medical technology, stent intervention has become one of the very important treatments for coronary heart disease, but stents are not all-powerful and can also cause allergic reactions, complications and other problems, so we invite Director Zhou Zhongjiang to talk about his views on interventional stent treatment for coronary heart disease, hoping to provide help for the majority of patients. When should coronary angiography be performed? When the diagnosis is clear, when the symptoms of angina cannot be effectively controlled and relieved by medication, when the symptoms are unstable, when there is frequent chest pain, and when the effect of medical antiplatelet therapy and anti-myocardial ischemia therapy is not obvious, an invasive coronary angiography examination should be performed in a timely manner to determine whether interventional stent treatment for coronary artery disease is needed according to the situation shown by coronary angiography. What is the exact cost of stenting? The cost depends on the situation. Generally, if the lesion is relatively short and simple, less devices are used, and there is only one drug stent, in China, the national pricing of drug stent is 12,000 yuan, plus balloon and angiography, it costs about 30,000-40,000 yuan, depending on the region, and if a bare stent is put in, it is about 6,000-8,000 yuan. What should I do if a patient has allergies from stent implantation? First of all, we have to rule out that the allergy is caused by other reasons. Allergy to stents, especially nickel and gold allergy, is not uncommon in foreign countries during study abroad and has not been given enough attention in China. After stent implantation, if a patient develops skin allergy or airway hyperreactivity, we should first rule out that the patient has allergic diseases, such as bronchial asthma, and of course, we should also rule out whether it is related to the medication taken after stenting. If stent allergy is diagnosed, only anti-allergic treatment, such as anti-allergic treatment with a small amount of hormone or other drugs, can be administered, because the stent cannot be removed after implantation. When should I consider restenosis after stent implantation? If there is a certain period of stability after stent implantation, usually after nine months, myocardial ischemia, electrocardiogram st segment, ischemic changes, chest pain, etc., it is necessary to consider whether there is restenosis of coronary arteries, and coronary angiography is needed for review. What about restenosis after stenting? The incidence of stent restenosis is relatively low for drug stents, about 7% or less, and the incidence of restenosis is less than 10% for super drug stents, which are better studied. In general, restenosis is not a major problem. What are the complications of stenting? Stenting is a type of interventional surgery and has the same risks as other procedures: from allergy to local anesthetic drugs, anaphylaxis to contrast agents, to intraoperative complications of local vascular embolism, to perforation of cardiac vessels due to interventional devices, to transient myocardial ischemia due to balloon dilation and stent implantation, to arrhythmias and ventricular fibrillation, etc. In general, the incidence of complications in mature interventional centers is relatively low. The incidence of complications is relatively low and there are corresponding management plans.
