Shoulder pain is not always caused by “frozen shoulder”

  ”Frozen shoulder is a medical condition known as frozen shoulder. It is mostly seen in middle-aged and elderly people in their 50s, and is also commonly known as the “50th shoulder”. When you feel pain in the shoulder joint or around the shoulder in your daily life, “frozen shoulder” is often mentioned in the mouth of the people, as if shoulder pain or discomfort is caused by “frozen shoulder”, which is actually a misconception. Even orthopedic surgeons who are not trained in shoulder disorders may have misconceptions. Shoulder pain can be caused by shoulder disorders, but it can also be caused by extra-articular disorders such as angina pectoris, cervical spondylosis, or localized manifestations of systemic diseases such as rheumatoid arthritis. The first task for patients with shoulder pain is to find the cause, not to get lucky and think that it is “frozen shoulder”, so as not to delay treatment.  Shoulder joint pain caused by shoulder joint diseases include: 1. Rotator cuff injury
The incidence is the highest, accounting for 30%-40% of shoulder joint diseases. Typical symptoms are pain in the neck and shoulder at night and pain in the arm when lifting; sometimes they are afraid to sleep on the affected side and even wake up with pain; the shoulder joint can be weak when abducting, lifting or posterior extension, and sometimes there are difficulties even in personal hygiene, which seriously affects the patient’s life. An MRI of the shoulder joint can reveal rotator cuff discontinuity or tear. Rotator cuff injury is usually treated conservatively, but for those who have a complete rupture of the tendon, surgical treatment should be used, and arthroscopic minimally invasive repair of the rotator cuff is effective and less invasive.  2. Rotator cuff impingement is second only to rotator cuff injury and is most commonly seen in patients with repetitive over-the-top sports such as throwing athletes. It is often combined with rotator cuff injury. The pain often radiates to the proximal lateral and middle part of the arm, and the symptoms increase when throwing or lifting the arm. Subacromial closure is effective. If treatment is delayed, the patient may experience severe muscle atrophy and sleepless nights; a hooked shoulder can be seen on x-ray. Combined rotator cuff injury is seen on MRI.  After diagnosis, active conservative treatment such as medication, physiotherapy, functional rehabilitation, and lifestyle changes should be undertaken. If conservative treatment is still ineffective, minimally invasive arthroscopic surgery can be performed, and acromioplasty decompression is effective and quick.  Shoulder instability, also known as habitual dislocation of the shoulder joint, is the third most prevalent shoulder joint disease. Due to trauma or degeneration of the joint structure, as well as the high mobility and relatively poor stability of the shoulder joint itself, the shoulder joint is prone to dislocation or subluxation. The affected shoulder will produce pain, impaired movement, and limited function. CT
or MRI can confirm the diagnosis. If the shoulder is not treated in time, bone loss and joint surface destruction will occur, making the later treatment difficult and in some cases, even tricky.  4.Frozen shoulder
It accounts for only 17% of shoulder joint diseases. It is characterized by pain in the shoulder joint, which is heavy at night, and is called frozen shoulder because of the restricted movement of the shoulder joint. Treatment includes medication, physical therapy, functional rehabilitation, intra-articular injection, etc. The duration of the disease is about 2 years.  5. Other diseases such as SLAP injury, tenosynovitis, synovitis, calcific myositis of the supraspinatus muscle, osteonecrosis of the humeral head, acromioclavicular arthritis, etc.  Therefore, it is recommended that patients with shoulder pain should go to the orthopedic department or sports medicine department of the hospital for consultation to clarify the cause, treat it early, relieve the pain and return you to a healthy body is the greatest wish of medical workers.
