The onset of the disease is characterized by a sudden onset of persistent penile erection for a few hours or even weeks without sexual stimulation, accompanied by swelling and pain in the penis, and in some patients, difficulty in urination or urinary retention. In Chinese medicine, this disease is called “Yang Qiang” and “Qiang Zhong”. The disease is mostly seen in young adults, and its onset is rare. The etiology of this disease is clinically divided into primary and secondary. The etiology of primary abnormal penile erection is not clear, but some cases are related to continuous sexual stimulation, such as prolonged sexual intercourse, repeated masturbation and application of aphrodisiac drugs. All four cases in our hospital were related to the use of aphrodisiac drugs. Secondary abnormal penile erection can be caused by a variety of etiologies, such as leukemia, sickle cell anemia and other blood disorders, infectious myelitis, multiple sclerosis, spinal cord tumors and other neurological disorders, penile or urethral injury, pelvic tumor compression, prostatitis, urethral stones, encopresis, and the use of guanethidine, 6-amino acid, heparin, poppy bases, phentolamine and other drugs. All may cause the above symptoms to occur. 2, Chinese medicine etiology pathogenesis “Yang strong” or “strong in” is mostly due to emotional discomfort, liver depression into fire, fire burning the tendons, resulting in tendons body detention; or masturbation without control, excessive sexual intercourse, do not know hold full, semen long ejaculation, depletion of true yin, Yin deficiency Yang hyperactivity, so that the tendons vibration, and the stem body The strong and hard body does not decline; or long-term qi stagnation, stems lifting too long, bruises and injuries caused by blood stasis Yang complex, resulting in Yang strong not fall. In the early stage of the disease, liver fire, liver and gallbladder heat and Yin deficiency and Yang hyperactivity are more common; in the middle stage, phlegm, dampness, stasis and blood obstruction are the main causes; in the late stage, due to Yin loss and Yang, Qi and blood deficiency, can lead to impotence. This disease should be distinguished from hypersexuality. In hypersexuality, the erection of the penis is influenced by sexual desire and is satisfied by sexual pleasure, and can be softened immediately after the discharge of semen. In contrast, abnormal penile erection is not affected by sexual desire and does not loosen up after the discharge of semen. C. Treatment 1. Western medical treatment (1) non-surgical treatment: sedative painkillers, estrogen, etc. can be used according to the specific condition, and general anesthesia with ketamine, lumbar anesthesia, epidural anesthesia, etc. can also be used to interrupt the erectile response. (2) Manual treatment: After incision of the penile corpus cavernosum and removal and washing of the accumulated blood in the corpus cavernosum, the penile corpus cavernosum and urethral corpus cavernosum blood shunt can be obtained. (3) Puncture and aspiration of flushing fluid: use a 12-gauge needle with a 50ml syringe and flush repeatedly with saline until there is fresh blood flow and the penis is flaccid. And the penis that has been loosened is repositioned in the downward state, and pressure bandage is applied. In this paper, four patients were treated with this method plus Gentian Diarrhea Liver Soup plus reduction, with good results. 2, Chinese medicine diagnosis and treatment (1) liver and gall bladder damp heat: the main evidence for the stem does not fail to lift, swelling and hot pain. Remedy: Zhi Bai Di Huang Wan plus or minus. 3, mutual obstruction of phlegm and stasis: standing evidence of stem lifting for a long time, stem color is purple and dark, wood-like swelling and hardness. White and greasy tongue coating, slippery pulse. Treatment: treating blood, relieving dampness, resolving phlegm and clearing ligaments. Prevention and prognosis: The prevention of abnormal penile erection should actively treat the original disease, such as prostatitis, bladder, posterior urethral stones; avoid injury to the penis or urethra; use less or no drugs that can cause the disease, abstain from sex, and treat the disease promptly after the onset. In the process of treatment using a combination of Chinese and Western medicine, pay attention to the reconciliation of yin and Yang, the recovery of sexual function and protection and promotion.
