Chinese medicine has a long history of understanding hemorrhoids, as early as the oracle bone inscriptions of the Yin and Shang dynasties, there are records of anorectal diseases. During the Western Zhou period, the name of hemorrhoids was recorded, and during the Spring and Autumn and Warring States periods, a taxonomy of hemorrhoids, surgical treatment and drug therapy were proposed. The Fifty-two Diseases Formula first described in detail the symptoms of hemorrhoids and the ligature excision of hemorrhoids. Subsequently, the Shennong Ben Cao Jing first recorded 21 types of drugs for hemorrhoid fistula. During the Qin and Han dynasties, the earliest knowledge of the etiology of hemorrhoids was found in the Yellow Emperor’s Classic of Internal Medicine, and during the Jin dynasty, the first detailed acupuncture treatment for hemorrhoids was recorded in the Acupuncture and Moxibustion Classic. In the Sui Dynasty, there was a great development in the understanding of the etiology of hemorrhoids. Chao Yuanfang’s “Treatise on the Origin of Diseases” believed that all hemorrhoids were caused by wind, indiscretion in the room, drunkenness and fullness, which disturbed the blood and caused the flow of blood through the meridians, leaking between the intestines and rushing to the lower part of the body. In the Tang Dynasty, Sun Simiao introduced the treatment of hemorrhoids with hedgehog bark and carp intestine in the Prepared Urgent Thousand Gold Essential Formula and Thousand Gold Wing Formula. In the Song dynasty, the treatment of withered hemorrhoids was recorded in more detail, including the treatment of withered hemorrhoids and withered hemorrhoids nails, and the treatment of spider silk ligation of hemorrhoids was proposed. In the Jin-Yuan period, there was a great progress in the understanding of the etiology of hemorrhoids and the treatment of hemorrhoids, and it was believed that the etiology of hemorrhoids was based on dampness, heat, wind and dryness, and the treatment rule of clearing the heat, dipping the dampness, draining the wind and moistening the dryness was established. The understanding and treatment of hemorrhoids in Chinese medicine was more comprehensive and mature in the Ming and Qing dynasties. The Ming Dynasty “Surgery Qixuan” is divided into 24 hemorrhoids, including dirty carbuncle hemorrhoids, locked anal hemorrhoids, lotus hemorrhoids, internal and external hemorrhoids, plum hemorrhoids, walnut hemorrhoids, pomegranate hemorrhoids, and chicken heart hemorrhoids. From these disease names, we can see that the division of hemorrhoids was mainly based on the experience of medical doctors and was relatively intuitive, mostly by morphology. In addition, it is a good idea to take into account the fact that the blood vessels are not working due to prolonged sitting, and that the blood vessels are not working due to the seven emotions, as well as the fact that the blood vessels are crossed and the meridians are intertwined due to the heavy load, or the fact that the intestines and stomach are injured due to excessive drinking and sex, so that the blood and stagnant gas flow into the anus, which can cause hemorrhoids. This is the case for both men and women, old and young, and there are piles that are born within the anus and protrude outside the anus.” In terms of treatment, different treatment methods were proposed for internal and external hemorrhoids in different stages. In the late Ming and early Qing dynasties, the preparation, use, post-use observation, and prevention of arsenic poisoning were all perfected in the preparation of hemorrhoid medicine. At this time, withered hemorrhoid therapy had become the main method of hemorrhoid treatment in Chinese medicine. Since the 1950s, the research on hemorrhoids has been greatly developed. The research on the therapy and the treatment mechanism of Kui Haemorrhoid San and Kui Haemorrhoid Nail was carried out successively, and the treatment principles of Kui Haemorrhoid San and Kui Haemorrhoid Nail were revealed. In clinical practice, the “Arsenic-free hemorrhoid nail” is widely used and has achieved more satisfactory therapeutic effects. On the basis of this, we have widely carried out the research of internal hemorrhoid withering injection and new sclerotherapy of sclerosing and atrophying therapy with the main component of hemorrhoid medicine. At present, injection therapy has become a major method of Chinese medicine in the treatment of hemorrhoid disease. In terms of surgical treatment many procedures have been derived based on the traditional ligation surgery, but the common one at present is still internal ligation and external peeling; with the deepening understanding of the nature of hemorrhoids, the progress of hemorrhoid therapeutics is also remarkable, and the PPH procedure, which is now widely accepted and carried out, does not have a long history even in foreign countries. In 460-370 B.C., hippocrates, who is regarded as the father of Western medicine, pointed out that hemorrhoids were the accumulation of waste products from spleen blood and bile, and that bleeding hemorrhoids were the excretion of these accumulations. He believed that the excretion of hemorrhoids could prevent pleurisy, pimples, abscesses, and epilepsy. In the 17th century, Professor Stahl, the Prussian court physician at the time, still proposed that “hemorrhoids are a live valve of the portal system that regulates blood, a kind of safety valve when the body has too much blood, and when they bleed A kind of self-cleaning” phenomenon of life rather than a disease. This concept is deeply rooted and widely spread in Western folklore, such as the German term hemorrhoids as “golden veins goldendaer” and the old French term for the flow of gold flaxd’or the old Italian term for the flood of blood profluvio disangue. This concept has long curbed people’s enthusiasm for the exploration of hemorrhoids, affecting the research and development of hemorrhoid therapeutics, so that people are helpless to do anything about the symptoms of hemorrhoids. Medieval Europeans had to wear dried toads and charms one to treat anal bleeding. They prayed for the blessing of ST Fiachra, the patron saint of hemorrhoids. Martin Luther, the religious reformer, and Richelieu, the chancellor of Louis XIII, complained about the torment of hemorrhoids and the incompetence of doctors in their long letters to their friends. The Austrian admiral Don John did not die on the battlefield against the Turks, but died of hemorrhoid hemorrhage. Napoleon, the French emperor, lost his entire army in the famous Battle of Waterloo because of a hemorrhoid attack. From the Western text, hemorrhoids in English is called Hemorrhoid, this word from the Greek, Hemo – is the meaning of “blood”, -rrhoid is the meaning of “flow like”, together with the meaning of The word Hemorrhoid comes from the Greek, Hemo- means “blood” and -rrhoid means “flow-like”, together they have the meaning of “blood flow”. This shows that hemorrhoids have been considered to be a bleeding disorder since ancient times. The narrow concept of hemorrhoid is a hemangioma formed by stasis, flexion and dilation of the arteriovenous plexus in the lower rectum, anal canal and anal area. These hemangiomas are soft and flexible. As can be seen, the Chinese definition of hemorrhoids is much broader than in the West, and any unevenness in or around the pan-anus can be considered hemorrhoids, such as inflammation of the surrounding skin, edema or subcutaneous thrombosis, and even connective tissue sagging or flaps are included in the category of hemorrhoids. Statistics show that 71.1% of so-called hemorrhoid patients have severe or mild symptoms, while 28.9% have no clinical symptoms, indicating that there are a large number of asymptomatic hemorrhoid patients. Modern belief is that hemorrhoids are a normal part of the body and do not need to be diagnosed and treated; they are considered a pathological condition only when they develop significant symptoms that affect the quality of daily life, and only then do they need to be diagnosed and treated. Patients with hemorrhoids should be defined as i.e. those who have symptoms and cause harm and pain to the body.
