Meibomian gland dysfunction (MGD) is an ocular surface disorder that is very common in oily skin and older individuals and is a major cause of evaporative hyper-evaporative dry eye. Recent studies have shown that in the early stages of MGD, the lipid composition of the secreted lid glands is abnormal, as evidenced by increased free fatty acids that form foam affecting tear film stability, decreased wax esters, and elevated cholesterol esters that increase viscosity blocking the ducts, thus providing the substrate needed for bacterial colonization. Some studies have found that cholesteryl esterase and fatty wax esterase from Staphylococcus epidermidis can break down lid gland lipids, forming metabolites that irritate the lid margin and exacerbate ocular discomfort in patients with MGD. MGD patients often suffer from tear deficiency, leading to tear film instability, accelerated tear film evaporation rates, and increased tear osmolarity. Additional risk factors for MGD include lupus erythematosus and rosacea. Common symptoms include redness, itching, dryness, photophobia, tearing, burning sensation, foreign body sensation, fluctuating vision, visual fatigue, and other symptoms of eye discomfort. Obstruction of the lid gland opening is a common cause of lid gland dysfunction. Obstruction of the lid opening can directly or indirectly cause changes in the quantity and quality of lid gland secretion, resulting in unstable pressure on the tear film, and a decrease in lipids can cause faster evaporation of tears leading to ocular surface dryness. On examination of the eye, the lid margin is seen to be congested, irregular, thickened, with a blunt posterior margin and scaly eyelash roots; the lid gland orifice is prominent, displaced, with poorly defined borders, dilated surrounding capillaries, and obstructed by yellow lipid secretions, with yellowish-white, granular, or toothpaste-like secretions overflowing from the lid gland orifice when the lid margin is squeezed. Other common accompanying signs include chalazion, conjunctival calculi, conjunctival congestion, papillary hyperplasia, punctate staining of the cornea, and in severe cases, corneal vascular opacification, corneal ulceration, and ectropion. Treatment: 1. Clean the lid margin: Use a 3% boric acid solution, or 0.9% sodium chloride injection to clean the local lid margin and eyelashes. If there is secretion knotted in the lid margin at night, cleaning the eyelid in the morning is more effective. 2, eye hot compress: lid gland blockage can be hot compress, the xuanming powder 15 ~ 20g into the washbasin, pour about 500ml of boiling water, put a cotton towel, wait for the temperature of about 37 ~ 400C (lid grease at room temperature is liquid, the melting point of 19.5 ~ 32.90C.), wringing, folded into a square, the patient closed the eyes, the original hands after the towel placed in the eye hot compress, wait for the towel to cool, replace 1 ~ 2 times. 3. Squeeze the lid: apply a hot compress to the eyelid for 3 to 5 minutes to soften the lid gland secretions, then place a finger on the skin surface of the eyelid relative to the lid gland, rotating it while pushing in the direction of the lid margin to discharge the secretions. 4, eye drops: antibiotic eye drops: choose levofloxacin eye gel, ciprofloxacin, bacitracin, clindamycin eye drops; diclofenac sodium eye drops or tobramycin dexamethasone eye drops 3 times a day for 1 to 2 weeks (pay attention to intraocular pressure); together with sodium vitrate, or polyvinyl alcohol eye drops. 5, oral treatment of Chinese medicine: Huang Lian 5g stiffness 10g Poria 10g Fa Han Xia 10g Chen Pi 6g Licorice 3g. eye burning, mouth bitterness and dry throat plus Scutellaria 10g Xuan Shen 10g; eye itching discomfort plus Cicada 6g Thornbush 10g Fang Feng 10g Di Shen Zi 10g Chrysanthemum 10g; eye dryness and astringency plus Xuan Shen 10g Madonnas 10g. 1 dose daily, 2 times with water. 6, prevention and conditioning: keep the eyes clean, avoid wind, sand, smoke and dust irritation; pay attention to dietary regulation, do not eat too much spicy, do not drink alcohol.
