It is an inflammatory reaction of the flexor tendon sheaths and tendons of the fingers due to chronic injury. The main manifestations are pain at the root of the finger and a popping sound when the finger moves. The pain is often severe and causes a lot of pain to the patient. Among them, occurring in the thumb is called popping thumb; occurring in other fingers is called popping finger. 1, the pathogenesis of the hand flexor tendons in the finger tendon sheath, finger flexion and extension tendons in the tendon sheath slide. Repeated, prolonged forceful activities of the finger can cause damage to the tendon sheath and tendon due to friction, resulting in edema and thickening of the tendon and thickening and narrowing of the tendon sheath. The edematous tendon is pressed into a gourd shape by the tendon sheath, and when the finger is flexed and extended, the gourd-shaped thickened tendon will pop when it passes through the entrance of the narrow tendon sheath. Knitting sweaters and using scissors are the most common triggers. Therefore, continuous knitting sweater or cutting is prone to this disease. 2.Clinical manifestations The disease is mainly characterized by pain and ringing at the root of the fingers. At the beginning, the stiffness and pain of the fingers are felt in the morning, but disappear after activity. Later, the pain and ringing in the morning will disappear only after a period of activity. In severe cases, the pain is obvious and does not ease throughout the day. In addition to snapping, the fingers may also become atretic and cannot be extended or flexed. A small hard node can be palpated at the root of the finger, which is painful to press. All five fingers of the hand can be affected. However, the thumb, ring finger and index finger are more common. The incidence is higher in the right hand than in the left hand, and can be single or multiple. 3, treatment First take conservative treatment. First of all, let the affected finger rest and reduce the activities of the affected finger to help eliminate inflammation and edema. Oral medications such as anti-inflammatory pain and ibuprofen can be taken to reduce inflammation and pain. Physical therapy can be performed. In the acute stage, massage can aggravate the injury and should be noted. Cortisone acetate intra tendon sheath closed injection is more effective. After the closed injection, the pain is relieved, but it should not be thought that it has been cured, because the inflammation and edema have not completely disappeared, and it is easy to recur if the finger is not rested. In addition, hyaluronic acid can also be injected into the sheath to nourish and lubricate the tendon. Physical therapy is effective in reducing swelling and pain. With the above treatment, most patients can be cured. A small number of patients should undergo surgery if conservative treatment is ineffective or if there are multiple recurrences. Surgery is performed under local anesthesia, with a small incision and little damage. During the operation, the tendon sheath will be cut at the beginning of the narrowing and thickening, and the ringing will disappear. 4.Prevention Most of the disease is caused by long and fast finger labor. The middle-aged and elderly people should not work with their fingers for too long in daily life, and pay attention to the prevention of finger tendon and tendon sheath injury. Pay attention to intermittent rest and relaxation of the fingers during labor. When finger stiffness occurs, even if there is no pain, you should stop working in time and take proper rest to prevent the development of snapping fingers or snapping thumbs. The disease is also related to systemic factors. It is easy to develop after illnesses such as heavy colds and fevers or when there is a sudden change in weather. At this time, middle-aged and elderly people should reduce the intensity of hand work to prevent tenosynovitis.
