Tendovaginitis Introduction Stenosing tenosynovitis of the flexor tendons, also known as “ringing finger” and “trigger finger”, is a common clinical disorder of chronic soft tissue injury in the hand, which can occur in any finger, preferably in the thumb, finger, middle finger, and a small number of patients can have simultaneous onset of multiple fingers, usually in computer operators, manual laborers (carpenters, packers, weavers, and accountants). Clinical manifestations: the affected finger is limited in voluntary flexion and extension activities, and when the finger is flexed and extended with force, there is pain and a bouncing movement, such as pulling a trigger, accompanied by popping sounds. In severe cases, tendon inlay may occur, the affected finger is passively fixed in the position of flexion or extension, unable to move on its own, requiring the help of the healthy hand in order to move, the symptoms are aggravated in the morning and after labor, and can be relieved by activities or hot compresses. Pathogenesis: The tendon sheath of the flexor tendon is composed of a layer of fibrous sheath tube around the periphery of the flexor tendon, i.e. tendon sheath; and a bony sheath tube composed of a bony shallow groove on the palmar side of the metacarpophalangeal joint and transverse fibrous tissue localized in the tendon sheath of the flexor tendon, i.e. tendon sheath sliding car together. It plays a restraining and protective role for the finger flexor tendons. When the patient engaged in long-term hand labor or local pressure, the flexor tendon and tendon sheath repeated friction, extrusion of local congestion, edema, followed by local degeneration, adhesion, so that the tendon sheath local narrowing, the formation of buttonhole. Finger flexor tendon is squeezed and thinned, and both ends are enlarged into a gourd shape. When the finger flexion and extension activities, the tendon expanded part of the tendon sheath through the narrow slide, then the finger bouncing action, and cause pain, resulting in the affected finger flexion and extension restrictions. When the tendon sheath slide degeneration, adhesion is more serious, it will cause the finger flexor tendon embedded, the affected finger passively fixed in the position of flexion or extension, difficult to move. Commonly used treatment methods: most of the local external or with internal blood anti-inflammatory drugs; or the lesion of the local closed injection of anti-inflammatory; or take local physical therapy, hot compresses, acupuncture, massage and other treatments, although there is a certain degree of efficacy, but individual differences, and the efficacy of the treatment is not certain, it is easy to relapse. In the past, for patients who had incarceration of the flexor tendon or had typical “trigger finger” signs when moving their fingers, most of the patients were treated with surgical resection of the tendon sheath of the flexor tendon and tendon release surgery. However, there are problems such as greater surgical damage, longer recovery time, and scarring of the surgical incision. Pain according to the finger flexor tendon local anatomical characteristics, in-depth study of its pathogenesis, their own design and development of successful “tendon sheath bursa release push cutter”, after more than 2,000 cases of clinical application, so that all patients with stenosis of the lesion site immediately get completely loosened, the affected finger flexion and extension of free immediately after treatment, the pain soon disappears, an average of 7 to 15 days! The disease is cured, rarely recurrence, one-time cure rate of more than 98%, and rarely complications, low cost, the incision is only about 3mm, bleeding is very small. This technology won the Second Prize of Scientific and Technological Progress of Beijing Municipal Administration of Traditional Chinese Medicine for its advantages of minimally invasive, sure curative effect, high safety and easy to be popularized, and the external model of the tool is registered by patent and protected by the Intellectual Property Office of the People’s Republic of China.
