Recently, a 28-year-old woman with “headache, fever and cough for half a month” was admitted to our hospital. The patient was unconscious on admission and was diagnosed with severe systemic tuberculosis: hematogenous tuberculosis, tuberculous meningitis and tuberculous peritonitis. The patient’s consciousness was restored, but he had a severe headache, blurred vision, was bedridden, and could not pass stool. More than half a month has passed, but the patient’s condition has not fundamentally improved. This patient had 5 IVF sessions last year due to infertility, spending 100,000 dollars, and was pregnant for more than 5 months, but unfortunately miscarried in June this year. And now she has sold her property to cure her disease. As you know, tuberculous meningitis is a disease with a very high mortality rate, and the treatment course is about 2 years, and the cost of treatment is extremely high. The causes of infertility in Chinese women are greatly related to the infection of tuberculosis. Many women of childbearing age are infected with tuberculosis and are not actively treated because their organism has normal immunity at that time and does not show symptoms of active tuberculosis for a long time. It is possible that many women’s infertility is due to tubal tuberculosis leading to ovulation failure. The use of chorionic gonadotropin is required during IVF egg retrieval to repeatedly stimulate ovulation and the daily use of progesterone or chorionic gonadotropin after transplantation, and these hormones may lead to endocrine disorders in the body. Since egg retrieval is rarely successful at one time, repeated stimulation of ovulation many times leads to low ovarian response, predisposes to premature ovarian failure and may lead to ovarian hyperstimulation syndrome, which decreases the body’s immunity. This patient has done IVF 5 times and repeatedly used hormones, which leads to endocrine disorder of the organism, and the high nutritional demand of pregnant women during pregnancy may lead to malnutrition. The miscarriage aggravated the endocrine disorder, leading to a more depressed immunity of the organism and the spread of tuberculosis, resulting in severe systemic tuberculosis. I have treated more than 10 cases of this kind, all of which have developed tuberculous meningitis, either during pregnancy or after miscarriage or delivery. In particular, tuberculous meningitis that occurs during pregnancy is the most difficult to treat and is aggravated by changes in intracranial pressure during induction of labor. I once treated a pregnant woman who was 5 months pregnant with twins and had done IVF for 3 times, but the patient had to be induced due to tuberculous meningitis. Therefore, mothers who want to do IVF are advised to take good care of their bodies and be thoroughly checked for TB infection or TB and immune deficiency before doing so, so as not to spread TB or contract TB during the process of IVF, which may even endanger their lives.
