Should I have surgery for a middle ear infection?

Otitis media according to the different causes and course of the disease, can include: acute secretory otitis media, acute suppurative otitis media, chronic suppurative otitis media and other types of diseases, each type of treatment is different, whether the need for surgery should be analyzed on a case-by-case basis.
1. Acute secretory otitis media: it is mostly caused by factors such as dysfunction of the Eustachian tube, infection and allergy, which leads to negative pressure and accumulation of fluid in the middle ear cavity and produces symptoms such as tinnitus, stuffy ears and hearing loss. When timely and reasonable conservative treatment is ineffective, tympanocentesis, tympanotomy, tympanic tube placement and eustachian tube balloon dilatation are often needed.
2. Acute suppurative otitis media: part of the tympanic membrane is intact and pus accumulates in the middle ear cavity, tympanotomy can be performed at the right time for smooth drainage.
Indications for tympanotomy are: ① systemic and local symptoms are more serious, the tympanic membrane is obviously inflated, although the treatment is not significantly improved; ② the tympanic membrane has been perforated, but the perforation is too small, drainage is not smooth; ③ there are complications suspected, but do not need to immediately perform mastoidectomy.
3. Chronic suppurative otitis media: the main cause of this disease is acute suppurative otitis media, the indications for surgery are as follows:
(1) The middle ear has granulation or polyp, or the electro-otoscopy does not see obvious granulation or polyp, but the regular drug treatment is ineffective, CT shows that the mastoid process, the upper tympanic chamber and other lesions, the mastoid pathway tympanoplasty should be done.
(2) The middle ear inflammation has been completely absorbed, leaving the tympanic membrane tension central perforation, simple tympanoplasty is feasible.
There are other types of otitis media, such as cholesteatoma of the middle ear, which require surgical treatment. When suffering from otitis media, you need to actively seek medical treatment, systematic examination, clear diagnosis and condition, and follow the doctor’s instructions for standardized treatment.
