The Ex-PRESS atrial drainage implant is a new type of extraocular drainage procedure that is made of stainless steel and has excellent histocompatibility. Consisting of a 2-3 mm long, 0.4 mm diameter tube with an internal diameter of 50 microns, it can be inserted directly into the anterior chamber through the lamellar sclera under the scleral flap at the corneal limbus, connecting the anterior chamber to the subscleral space and the subconjunctival space. Its distal end is punctured into the anterior chamber, while the proximal end is located under the scleral flap, and the drain provides control of intraocular pressure by allowing a restricted outflow of atrial fluid into the subconjunctiva. In this way, external glaucoma filtration can be accomplished without the need to cut trabecular tissue and the iris, reducing intraoperative manipulation. In our clinical use, we found that in adult patients with open-angle glaucoma, intraoperative complications were significantly reduced with express drain implantation, and the postoperative long-term results were superior to those of trabeculectomy. Express drain implantation, a modification of trabeculectomy, still requires absorption of atrial aqueous through the filter bubble. express drains extend into the anterior chamber at the front end, which is less than 2 mm, but there must be some space. Surgical indications for Express drain implantation: Patients with an open atrial horn who are suitable for trabeculectomy can opt for express drain implantation.
