About the limitations of telemedicine

  As far as the burn orthopedic profession is concerned, there are some simple cases, or emergency situations, where telemedicine can sometimes provide some help in a timely and accurate manner.  However, more cases, but often relatively complex …… text narrative, or even more advanced photos, video, but not always provide enough information to determine the injury; not to mention, still need to be accurate, precise diagnosis based on the formulation of relatively detailed individualized treatment plan.  In other words, with the advancement of modern technology and the corresponding progress and refinement of clinical treatment measures, there is often a need for meticulous and highly targeted individualized treatment plans …… and the development of this plan requires more than just a detailed medical history and accurate examination of the specialist condition; for some orthopedic cases, it requires repeated communication between the doctor and patient. Through face-to-face communication and interaction: …so that the doctor can more accurately grasp the patient’s injuries, problems and urgent issues; the patient can also more accurately understand their own condition, evolution, different options for treatment measures and their relative advantages and disadvantages. In this regard, telemedicine, with the limitations of its communication medium, often does not provide enough information, not to mention that sometimes there are differences in the way of thinking due to geographical and professional differences, which may further cause inadequate communication.  Recently, a patient with a severe cervicothoracic scar deformity was hospitalized and returned to the hospital for a follow-up examination about one month after discharge; it brought us both a surprise and a thought on the issue of “return” and “in-person” follow-up… …Surprisingly, the surgical result was very satisfactory, and the patient’s local functional exercise, anti-scar treatment, and reshaping after discharge were carried out thoroughly and accurately according to the medical advice at the time of discharge (the boy was only 16 years old), and the local appearance and function were consolidated and restored to a considerable extent; the remaining problems were almost no longer “difficult”. The remaining problems are almost free of “difficulties”. …… During the specialist examination and communication with each other, some more details were found, and further functional exercises, anti-scar treatment, reshaping, etc. were slightly adjusted; and after the agreed time for further follow-up and the approximate duration of the later treatment, the patient returned happily.  In fact, all these, almost in just more than ten minutes, completed …… but, for these ten minutes, and more, only dozens of minutes, the patient and family, but by hundreds of miles of travel, the wind and dust, “special” “return to the hospital” “face to face” (face to face) with the consultation (in fact, the doctor’s phone number and other contact information, the patient has been left in the hospital before discharge.) …… thought-provoking is that, even now, technology, information network is so developed, thinking back, the patient’s situation, whether phone calls, text messages, photos, and even video, I’m afraid it is difficult to reach in this short ten minutes or so, mutual communication and access to the “amount of information “In fact, in the clinical treatment process, the above-mentioned patient’s condition, whether by phone, text message, photo, or even video, I’m afraid it is difficult to reach the amount of information that can be exchanged and obtained in just ten minutes. But, similar to the above case for many burns, plastic surgery …… In fact, the surgery itself, is only a very important part of the treatment; but, far from being the whole of the treatment. Among them, detailed history taking, accurate specialist examination, and accurate diagnosis on this basis are the basis for the formulation of treatment plans including surgery; while postoperative functional exercises, anti-scar treatment and even reshaping and other corresponding measures, as well as adjustments made depending on the specific situation, are often the consolidation and guarantee of treatment results. In this case, especially when there is a demand for “high quality” “medical results”, telemedicine is not always a substitute for “in-person” (or “Face-to-face” treatment is not always a substitute for “return to the hospital” or “in-person” follow-up.
