Chronic tonsillar enlargement medical case

  Liu Moumou, female, 24 years old, Lintao County, Gansu Province Initial diagnosis (May 29, 2009): The patient had chronic pharyngitis and enlarged tonsils for 2 years. Whenever she gets cold, she has sore throat and dry throat, and swallowing is especially bad. In severe cases, he had fever, sputum in the morning, and felt nauseous when brushing his teeth. A week ago, the above symptoms worsened without any obvious cause, and the symptoms were not relieved by taking “Cefadroxil and Banlangen”, so he came to our department today. He came to our department today with the following symptoms: sore throat, aggravated by swallowing, dry throat, nausea, a small amount of white mucous sputum, obvious in the morning, nasal and sleepable, dry stool, one line for three days, urine regulation. Physical examination: congestion in the pharynx, no follicles, bilateral tonsils Ⅲ° enlarged, about 1.5×1cm in size, soft, dark red in color. The tongue is light red, with white greasy coating and smooth pulse. Both lungs (-). Blood count: normal. Western medicine diagnosis: chronic pharyngitis; Chinese medicine evidence: breast moth. The evidence belongs to phlegm and blood stasis, the treatment is to activate blood circulation and disperse nodules, resolve phlegm and promote pharynx. Decocted twice with water and divided into two doses twice a day.  Second consultation (June 13, 2009): The patient’s sore throat and dry throat disappeared, no nausea, sputum volume decreased, easy to cough up, stool once a day, soft texture. Physical examination: congestion in the pharynx, tonsils I degree enlargement, dark red, soft texture. The tongue is light red, the coating is white and greasy, and the pulse is smooth. The effect does not change the formula. Followed by 7 doses.  Third consultation (June 20, 2009): The symptoms disappeared on follow-up. Physical examination: the pharynx is red and the enlarged tonsils have completely disappeared.  This is a case of “breast moth” in Chinese medicine. This is a case of “breast moth” in Chinese medicine, but the tonsillar enlargement in Western medicine. From the evidence, it appears that the evil Qi is not prevalent and the phlegm is the main cause. The cause is deficiency of positive qi, deficiency of spleen, stagnation of liver qi, internal production of phlegm, congestion in the lungs, long-standing treatment, formation of old phlegm, stubborn phlegm, congestion in the throat, long-term disease will be stasis, and eventually phlegm and stasis in the pharynx, resulting in swelling of the throat. In this period, we should focus on “dispersing phlegm, resolving phlegm, invigorating blood and moving qi”. The Nei Jing says: “Those who are depressed disperse them, and those who are knotted fight them.” But in the process of dispersing the knots, we should also focus on strengthening the spleen, tonifying the spleen, and transporting the spleen, in order to eliminate the source of phlegm. From Yu’s experience: if the tonsils are enlarged, if the texture is soft and the color is bright red, it means that the phlegm and stasis are light and easy to treat; if the texture is hard and the color is dark red or purple, it means that the phlegm and stasis are heavy and difficult to treat and disperse. If necessary, add insects to search the ligaments and activate blood circulation to break the stasis, in order to be effective.
