Neck and shoulder syndrome, also known as neck and shoulder myofasciitis, is a common disease in rehabilitation and orthopedic clinics, with an incidence of about 25% of the clinics. There were 28 patients, 11 males and 17 females, aged 26 to 64 years, with a duration of 3 months to 4 years. Patients had a clear history of cold or trauma to the neck and shoulder, with recurrent attacks, and more women than men. Most of the patients had unilateral neck muscle pain, chronic and persistent soreness and swelling, and pain with tightness or heaviness. The pain is triggered by local cold or exertion, cold stimulation, or even waking up at night with pain, stiffness and pain in the morning, and can only be relieved after activity, often aggravated after long hours of work, and aggravated by prolonged inactivity or excessive activity. Physical examination shows that the neck and shoulder muscles on the affected side are tense, and the muscles feel stiff when pressed. The pressure points are fixed, mostly near the starting and ending points of the muscles or at the intersection of muscles of different directions, and soft tissue nodules or painful strips can be palpated deep in the pressure points. In some patients, pressure pain can cause pain or numbness in the upper arm. The needle knife breaks through the skin vertically and quickly, the subcutaneous incision line is parallel to the longitudinal axis of the spine, and there are different degrees of breakthrough sensation when the needle knife breaks through the superficial and deep fascia, and the tense fascia is loosened by small-scale lifting and cutting near the needle entry point, and the longitudinal and transverse plugs are separated and cut off the adherent brachycephalic tissue and fascial hard knots. The muscle fibers are parallel to the direction of the fan-shaped longitudinal sparse transverse paddle muscle, feel a sense of loosening under the needle out of the needle. A small number of patients had significantly reduced neck and shoulder pain symptoms, although they still had discomfort after exertion or cold, but they did not need to take drugs for pain relief, and could be relieved by rest and adjusting work and rest habits. The overall treatment efficiency was 28 cases (92.8%), and 2 cases (7.2% ) were ineffective. All patients did not have anesthesia accidents and adverse reactions during the treatment, and no important nerves or blood vessels were damaged. Professor Zhou Xuelong, a national famous veteran Chinese medicine doctor and expert of Guangxi University of Traditional Chinese Medicine, pointed out that: cervical and shoulder myofasciitis is mostly caused by a sterile inflammation of local soft tissues after acute trauma to the soft tissues of the neck and shoulder or repeated chronic strain, which leads to prolonged muscle contraction due to inflammatory stimulation and ischemia, followed by muscle spasm. The prolonged muscle spasm and contracture may aggravate the aseptic inflammatory reaction of soft tissue, resulting in extensive and persistent fatigue and stiffness, pain, and unfavorable activity in the neck and shoulder, and in severe cases, pain and autonomic dysfunction in the neck and upper limb. Traditional treatment methods are effective but slow, with long treatment time and high recurrence rate. Acupuncture treatment can effectively reach the lesion and cut or peel off the adhesions or small nodules of the limited soft tissue. In Chinese acupuncture, “pain is the acupoint”, the acupuncture knife has both meridian stimulation and adjustment effects, and also has the modern anatomical separation of myofascial adhesions. Mechanical stimulation and separation improves local tissue metabolism, accelerates lymphatic circulation, and localized scar tissue is absorbed. The pressure within the soft tissues is reduced and the compression of the nerves by the surrounding tissues is eliminated. Acupuncture treatment for neck and shoulder syndrome has precise efficacy, low cost, little damage, and significant long-term effects.
