The diagnosis of thyroglossal cysts is basically confirmed by ultrasound combined with physical examination. If the mass is smooth and tough and the cyst retracts when the tongue is stretched out, it is very suggestive, because the lingual blind spot at the base of the tongue is connected to it, and if it moves with swallowing and is not with or in the thyroid gland, a thyroglossal cyst is highly suspicious. But one test that must be perfected is a nuclear thyroid scan, which aims to rule out ectopic thyroid. If it is cystic then a nuclear scan can barely be done. CT also has a better diagnostic value, plain scan can be, if the thyroid tissue density are higher, because of iodine. Treatment: surgical resection, relatively easy, local anesthesia can be general anesthesia. Thyroglossal cysts are predominantly male and occur in children and adolescents, with about 50% of cases occurring before the age of 20. The vast majority of patients can be seen in the anterior cervical mass, can occur in the midline of the neck from the tongue blind hole to the sternal incision between any part, but the most common upper and lower hyoid bone. The cysts are slow-growing, rounded and may be accompanied by localized symptoms such as neck distension, discomfort in swallowing, and a foreign body sensation in the pharynx. Combined with the infection can be manifested as a painful mass or abscess, if the fistula has been formed, visible sinus tract, sinus tract in the mucus or pus secretion outflow. Those with obvious infection may be accompanied by systemic symptoms such as fever and fatigue. Physical examination, in the neck near the midline can be palpable mass, soft texture, diameter 1 ~ 5cm, round or oval, smooth surface, clear boundaries, and the surface of the skin and surrounding tissues without adhesion, elasticity or fluctuating sense. In cysts located below the hyoid bone, tough cords can be touched between the hyoid body and the cyst and adhered to the hyoid body, which can be moved up and down with the movement of tongue extension.
