General principles of rheumatic disease treatment

  The treatment of rheumatic diseases, irregularly, is certainly a headache and a foot. It is no wonder, because the cause of rheumatic diseases is difficult to diagnose, which means that the diagnosis is quite difficult. However, we should still understand some basic principles: 1, only some symptoms of pain, and no other systemic disease manifestations, and for the time being, only symptomatic treatment.  2.If rheumatic diseases are considered to be potentially threatening, specialized treatment for the disease should be initiated promptly, such as the choice of hormones and immunosuppressants, the latter being mainly some slow-acting anti-rheumatic drugs with cytotoxic effects. Depending on the economic situation and the severity of the disease, one or two or three or four or even more drugs (e.g., proprietary Chinese medicine) are selected in order.  The second major category is based on systemic symptoms: such as systemic lupus erythematosus, polymyositis, and vasculitis, etc. Hormone therapy should be used mainly, supplemented with hydroxychloroquine. If there is visceral damage, immunosuppressants are added early. The optimal combination is prednisone + hydroxychloroquine + cyclophosphamide + leflunomide + cyclosporine + CD20 monoclonal antibody (also a biological agent). Depending on the economic situation and the severity of the disease, one or two, or even three or four, are chosen in that order.  The above combinations take into account the balance of economy, immediate and long-term efficacy, and safety.
