Working together to build a healthy defense

With the onslaught of the “aging tsunami”, China’s large population is the first to bear the brunt. From the patients admitted to our orthopedic department in the past 2 years, there are two trends. First, osteoporosis is getting younger; second, fracture patients are getting older. In October alone, there were more than 10 cases of fractures in our department with the age of 85 years or older. These elderly patients often have more comorbidities and poor cardiopulmonary function before the fracture occurs, which makes our treatment in a dilemma for a while. On the one hand, conservative treatment is prone to complications, causing chain reactions and life-threatening; for example, hip fracture in advanced age is called “the last fracture in life”, and long-term bed rest is prone to cause crushing pneumonia, lower limb thrombosis and even acute sudden death pulmonary embolism, with a mortality rate of more than 50%. On the other hand, surgical treatment is extremely risky intraoperatively and postoperatively due to the many preoperative comorbidities and poor cardiopulmonary base conditions of elderly patients. In response to the above problems, our department has developed a detailed treatment plan in conjunction with the intensive care unit under the premise of ensuring the life safety of the elderly patients. First, strict preoperative assessment of cardiopulmonary function and effective treatment of comorbidities; second, different surgical methods for individualized differences in patients, as well as preparation of various surgical contingency plans to prevent problems before they occur; third, postoperative transfer to intensive care, active medical treatment and monitoring, while strengthening the system of surgical check-ups. With the unremitting efforts of the physicians in our department and the guardianship unit, all the elderly patients were turned to safety and discharged healthily, which were always well received by the patients and their families. “The old man is as good as the old man, the young man is as good as the young man”, this is probably the most accurate prophecy of the ancients about the conduct of our profession. We cannot treat all patients as relatives, but our care is genuine and sincere; our love and care may not be perfect, but it is absolutely from the heart and with feeling. The unsoundness of the system and the imperfection of the service make the medical staff a target; the “Wenling incident” makes us feel disheartened and shudder. However, the harmony of society depends on its own tolerance, should not be “to the inch of rotten abandoned even hold the wood”, can not be because of the industry a little flaw, it is denied to the whole society’s great contribution. We are dissatisfied with the many medical at the same time, have you noticed that this profession will be the average life expectancy of less than 45 years old at the beginning of the founding of the country to the current 75 years; we have medical complaints about the expansion of the same time, have you noticed the wall of each department is full of banners. SARS died, is a hero; Wenchuan died, is patriotic; long-term radiation exposure to develop tumors, is dedication …… died under the knife of cruelty, is what! This is the sadness of the medical industry, but more worthy of the whole society, the whole nation to reflect. More safety measures can’t protect our lives; more government input can’t buy the glory of our profession. Expanding the scope of doctor-patient communication and strengthening the publicity of medical care itself became the last straw for us. After all, our fate is in our own hands. Doing our duty as medical professionals, we have no regrets!
