What if my bones don’t grow after a fracture?

  Any fracture that fails to achieve bony connection between the two ends of the fracture after 8 months of fracture is called osseous nonunion. The distinction between fracture healing and osteonecrosis is mainly determined by whether the interfracture material has ossified, and it has also been suggested that a fracture is osteonecrosis when it fails to achieve fracture healing at 6 months. The fracture is a common complication after fracture, also known as non-union, which is a condition in which the fracture end has stopped healing and the fracture end has formed a pseudo-joint. radiographs show that the fracture ends are separated from each other, the gaps are large, the bone ends are sclerotic, atrophic and lax, and the medullary cavity is closed. No amount of permanent fixation can make it join.  Bone discontinuity can be divided into hypertrophic, sclerotic and atrophic types, but most patients have a mixed type of bone discontinuity.  In hypertrophic bone discontinuity, the cartilage connection between the broken ends is mainly cartilaginous, and it is usually believed that the cartilage cap of the broken ends has the characteristics of epiphyseal cartilage, and the rate of cartilage generation and the rate of cartilage ossification form a dynamic balance, so the fracture gap exists for a long time. In hypertrophic osteoconnections, the fracture ends tend to expand to the sides, forming an “elephant’s foot” sign or a ball-like change.  In sclerotic discontinuities, the medullary cavity is closed and the fracture end is sclerotic.  Atrophic discontinuities have atrophied and thinned ends, mainly with fibrous connections.  Mixed osteochondritis often has both hypertrophy, sclerosis, or both atrophy and sclerosis.  In terms of treatment, hypertrophic osteoarthrosis is more treatable, while atrophic osteoarthrosis is more difficult to treat, especially if there is a pseudarthrosis.
