Combination of Chinese and Western medicine in tumor thinking and practice The following is a few perspectives of our combination of Chinese and Western medicine for your reference, and it is considered as a brick to draw in the jade. (1) Make full use of the anatomy of acupuncture points to use needles We should make good use of the anatomy of acupuncture points to guide the use of needles, which is the basis of our use of needles. Although the essence of meridians is still a mystery, the anatomy of acupuncture points can help us to think about the role of acupuncture points from multiple perspectives, as well as to guide needling and understand the contraindications of needling. You can refer to books such as “Anatomy of Acupuncture Points” by Professor Yan Zhengguo. We once treated a lung cancer patient with loss of appetite, and after using all the methods that usually work well, the patient still did not want to eat. What to do? The stomach is not moving, how to make it move? The stomach is like a long eye to avoid the new needles after entering the needles, and the stomach has to peristaltic movement to avoid them. I asked the student to stab around the patient’s stomach, and in less than a minute, the patient asked for food, and the effect was so fast that we described it as a “second kill”. (3) Using dorsal acupuncture points in relation to their nearby nerves to treat diseases of the internal organs: The dorsal acupuncture points of the bladder meridian correspond to the internal organs and provide great convenience in applying dorsal acupuncture points. Modern research has confirmed that the location of the dorsal points is adjacent to the spinal nerve root, which emits various spinal nerves, and the nerves emitted innervate the sympathetic and parasympathetic nerves of the internal organs of the thoracic and abdominal cavities, as well as certain parts of the trunk nerves. By stimulating the dorsal points, the nerve conduction can treat the corresponding internal organs or body surface disorders. Clinically, we have used blood cupping and fire acupuncture to treat related diseases such as asthma, coronary heart disease, intestinal obstruction, constipation, and gastroparesis with remarkable results. (2) Using pathophysiology to guide TCM evidence identification All of the students graduated from our college have studied pathophysiology, and it may not occur to you that we can skillfully use pathophysiology to guide TCM evidence identification. ①Use the formation of plaque in the vascular wall is related to lipid deposition, and inhibit the formation of blood vessels by removing phlegm and opening the ligaments: Those who have studied cardiovascular pathophysiology know that the formation of plaque in the vascular wall is related to lipid deposition in the vascular wall and damage to the vascular endothelium, but who can think that we can inhibit the formation of blood vessels by removing phlegm and opening the ligaments. We like to apply ten jujube soup to treat tumors related to phlegm, and after applying it for a period of time, ultrasound found that some patients’ tumors did not shrink but the blood flow inside the tumor disappeared, which precisely provides ideas for inhibiting angiogenesis R&D. Inhibiting angiogenesis does not need to focus only on blood vessels, but can start from inhibiting plaque formation, and the method of transforming phlegm and turbidity and opening up the ligaments and veins as described in Chinese medicine can inhibit angiogenesis. It also shows that the mutual existence of phlegm and stasis is an important tumor pathogenesis. The endometrial lining will change periodically with menstruation. When the endometrium is thickest is when the stagnant blood is heavier before menstruation, and when the stagnant blood is removed after menstruation, the endometrium becomes thinner. This shows that blood stasis exists in endometrial cancer (endometrial thickening). Clinically, triamcinolone acetonide tends to cause endometrial cancer. What conditions and what factors will go downward to cause endometrial cancer? Cold tends to go down and coagulate in the uterus, then it becomes blood stasis. Therefore, it can be concluded that triamcinolone acetonide is a cold medicine, and endometrial cancer is cold clotting and blood stasis. The theory of the sages more than 2000 years ago confirms that our present speculation is correct. (3) Chinese medicine identification by pathological pattern Small cell lung cancer and squamous carcinoma both have the factor of phlegm. In terms of pathological morphology, small cell lung cancer is granular and dense, while squamous carcinoma is more common as striated and scattered. Comparing to nature, those favoring cold like to gather in groups, while those favoring heat are scattered and arranged, then it can be said that fire is more important than phlegm in squamous carcinoma, and phlegm and fire are both important in small cell carcinoma. The nodules formed by pancreatic cancer are raised on the surface of the pancreas, which is analogous to the hard bumps in life, indicating that pancreatic cancer must be heavy on external cold, and at the same time there is internal heat holding phlegm and stasis. (4) According to the location of the tumor on the imaging to help the TCM identification Let’s look at lung cancer first. Lung cancer, whether small cell carcinoma or squamous carcinoma, is related to smoking, so it is on the fire side; those located in the periphery are on the cold side. The scattered nodules in the lungs are related to emotion and will. The CT film of thyroid cancer pulmonary metastasis shows scattered cotton wool-like masses. I deduced from this that patients with scattered lesions in the lungs are mostly related to emotions. By extension, herpes zoster, which looks like a string of pearls and develops rapidly, has the characteristics of wind; irritating cough, paroxysmal, also has the characteristics of wind, with Dai Clam San. Then look at the abdominal wall metastatic tumors: which tumors are prone to abdominal wall metastasis? Ovarian cancer is common, as well as bile duct cancer. Some of the metastatic tumors in the abdominal wall are similar to the manifestations of acute peritonitis. With the idea of treating acute peritonitis, we use Dazhi Chest Tang to treat metastatic peritoneal cancer with remarkable effect, so we can presume that metastatic peritoneal cancer favors phlegm and fire. It can also be speculated that tumors that easily cause peritoneal metastasis also contain phlegm and fire, in fact ovarian cancer and bile duct cancer mostly have phlegm and fire factors. (5) Relationship between site, pathology and yin and yang The site here is different from the imaging, which refers to the part that can be seen by the naked eye. According to Chinese medicine, “clear yang develops in the couples, while cloudy yin goes to the five organs” and “up is yang, down is yin”. In contrast, tumors located in the inner, lower and abdominopelvic cavities are mostly cold, and adenocarcinoma, mucinous and plasma tumors are more common. Take digestive tract tumors as an example, tumors of the oral cavity, pharynx and esophagus are often squamous carcinomas, while tumors below the cardia and up to the sigmoid colon become adenocarcinomas, and tumors of the anal canal become squamous carcinomas again. Adenocarcinoma of the endometrium is common, while tumors of the cervix, which is connected to the outside world, becomes squamous carcinoma. Some parts are not necessarily related to the pathology, but I still classify them according to the theory of Chinese medicine.
