After the middle of the 19th century, Western medicine was introduced to China in large numbers, with the arrival of missionaries, the translation of Western medical books, the establishment of Western medical schools and hospitals, and the absorption of foreign students, Western medicine squeezed the Chinese traditional medicine with an overwhelming force. The momentum of Western medicine was so strong and powerful that it almost monopolized the central and local health powers, and there was talk of abolishing Chinese medicine during the Republican period. At that time, the petition of the Chinese medicine community, led by Nanjing, shocked the whole country, and the four famous doctors in Beijing were created at this time. Chinese medicine was preserved with its unique and miraculous healing effects, but at times it was confronted with the searing and rapid development of Western medicine. In 1956, Mao Zedong came up with the idea of creating a new Chinese medicine based on the idea that the combination of the universal truth of Marxism-Leninism and the concrete practice of the Chinese revolution was the magic weapon for the victory of the Chinese revolution. He proposed, “Combine the knowledge of Chinese medicine with that of Western medicine to create a new unified Chinese medicine and pharmacology.” This is considered to be the original source of the concept of combining Chinese and Western medicine. Previously, in 1950, at the first National Health Conference held after the founding of New China, the “unity of Chinese and Western medicine” was established as one of the three major guidelines for health work, and a talisman for the preservation and protection of Chinese medicine was formulated. In 1954, Mao Zedong emphasized that “the most important thing in the future is that Western medicine should first learn from Chinese medicine, rather than Chinese medicine from Western medicine.” In this atmosphere, in 1955, the Ministry of Health held the first nationwide class for Western doctors to study Chinese medicine (hereinafter referred to as the “Western learning Chinese” class). Thereafter, the nationwide “Western learning Chinese” classes continued to be held systematically until about 1976, and various “Western learning Chinese” classes with or without release became an enthusiastic movement in the medical field. There is no denying that this period produced a number of very good experts in combined Chinese and Western medicine. In recent years, the domestic Chinese medicine universities set up a combination of Chinese and Western medicine, the purpose is also to strive for more people to do the combination of Chinese and Western medicine, master the knowledge of Chinese and Western medicine to treat diseases, but the fact is, the vast majority of doctors have become doctors who can simply apply Chinese and Western medicine, saying that they know everything, but when it comes to the integration of Chinese and Western medicine, we are at a loss, it is difficult to do the real integration of Chinese and Western medical knowledge, and can not combine Chinese and Western medicine. The early liberation of pure Chinese medicine to study the transformation of Western medicine, Chinese medicine scientific, so that Chinese doctors to learn the theory of Western natural sciences, such as anatomy, physiology, bacteria, pathology, etc.; Chinese medicine doctors scientific, that is, the examination of qualified Chinese doctors allowed to practice, through “further training” to modern medical transformation, so that they become “Scientific doctors”. Most of the Chinese medicine “directly replaced by Western medicine”, Chinese hospital leaders are Western medicine, the direction of development of Chinese medicine by Western medicine; and many new graduates of Chinese medicine students are increasingly Westernized, many hospitals of Chinese medicine even a basic prescription is incomplete, but under the guise of a graduate of Chinese medicine school, in fact, basically do not know Chinese medicine; many hospitals discuss Chinese medicine department Cases without TCM content; some hospitals set up TCM department is to grade review, has basically no TCM thinking, TCM treatment means. From the result, the combination of Chinese and Western medicine is the Westernization of Chinese medicine, Western medicine gradually replace Chinese medicine. In the face of the powerful Western medicine, we Chinese medicine is stagnant, or combined with Western medicine is every Chinese medicine people can not avoid the problem, I think but from the development of the history of Chinese medicine not only to combine, but also to seriously and properly combined. Chinese medicine was formed according to the philosophical ideas before the Warring States period, and was enriched and perfected by the scientific knowledge of that time in the trend of history, just like the formation of typhoid and warm disease doctrines, and now it is more important to enrich and perfect itself by drawing on modern scientific knowledge, and form a new Chinese medicine based on the inheritance of traditional medical characteristics. The combination of Chinese and Western medicine should be one of the mainstream of contemporary academic thinking in Chinese medicine, we Chinese medicine must not be stagnant, every sentence is not away from the internal classic typhoid, to be able to combine the internal classic typhoid knowledge with modern scientific knowledge to carry forward. So it seems that the development of the existence of modern medicine is not the road to the extinction of traditional medicine, the combination of Chinese and Western medicine is a stage in the history of the development of Chinese medicine, is the inevitable historical progress, no one can avoid. 2. Is Chinese and Western medicine really combined? Traditional Chinese medicine is “good at the exact theory, but poor at the physical”, while Western medicine is “specialized in the physical, but short at the exact theory”. Chinese medicine focuses on finding the law, macroscopic, Western medicine focuses on intuition, microscopic, how to combine macroscopic and microscopic organic complement each other is the way out of the combination of Chinese and Western medicine. The current combination is mostly a simple combination, not in the theory and methodology of organic combination will not produce a real combination. Acupuncture in foreign countries is also mainly to solve the pain, but it can have repeatability, tell him to use Hegu, cheek car to treat toothache, he put on even if the efficacy is not as good as you, but also can relieve part of the toothache patients, it is agreed, the West agreed is “repeatability”; pain is a disease? Not yet, it is just a symptom. So what should be the real high level combination of Chinese and Western medicine? I believe that the combination of Chinese and Western medicine should be based on the correct methodology, with a high degree of theoretical integration, which can guide the clinic or emerge new therapies, new ideas, or a 1+1>2 effect. However, it must be emphasized that the combination of Chinese and Western medicine can only be a partial integration and mutual interpretation, seeking common ground while reserving differences, and it is difficult to completely combine the two theories. The result of the combination of Chinese and Western medicine should be the excavation of Chinese medical theory, the development of academic thinking in Chinese medicine; at the same time to provide new thinking for modern medical theory, clinical provide new methods, identify the use of drugs to improve clinical efficacy. This is the only way to achieve the true integration of Chinese and Western medicine, to develop the academic of Chinese medicine, and to be recognized by modern medicine to draw nutrients, etc. 3. How to combine Chinese and Western medicine reasonably to study tumors (1) First of all, the names of Chinese and Western medicine must be interchangeable to the maximum extent. Previously, Chinese medicine talked about Wei-Yang and Ying-Yin together, and rarely talked about treating disease from regulating Wei-Yang alone. In TCM, most of the drinking evil is caused by Yang deficiency, so it is necessary to warm Yang and promote water. Therefore, the treatment of pleural fluid has both the western medicine interleukin-2, which has a warming effect on Yang, and the topical cream and warm needle therapy, which are used to warm Yang and fight cancer, and all of them have achieved obvious clinical results. It can be seen that because of the correspondence between pleural fluid and hanging drink, it is possible to clarify the reason for better treatment of pleural fluid with interleukin-2, and also provide ideas for the treatment of pleural fluid in Chinese medicine. The maximum interoperability of Chinese and Western medical names can provide great convenience for scientific research; and also provide the basis for screening effective treatment means in Western medicine, and provide ideas for exploring the treatment of modern diseases in Chinese medicine, etc. (2) must maximize the knowledge of Chinese and Western medical theory To achieve the integration of Chinese and Western medicine must maximize the knowledge of Chinese and Western medical theory, carefully analyze the similarities and differences, it is possible to achieve the integration of Chinese and Western medicine, put forward new theories, new ideas, to produce 1 + 1 > 2 efficacy. The following is the entry point of my new viewpoint for your reference: First of all, the white blood cells correspond to Wei Yang in Chinese medicine, as mentioned above. Then, can we promote white from the perspective of Wei Yang? This is why I came up with the idea of moxibustion of Guanyuan, Zhonggui, Shenqu, and Feosanli for the purpose of raising the white blood cells, and as expected, the white blood cells often rose to normal after 3 days. We believe that white blood cells have important defense functions, which can be divided into granulocytes and non-granulocytes. Among them, granulocytes play a very important role in the non-specific cellular immune system, while non-granulocytes mainly play a specific immune role. The external regulation of granulocytes is nothing but to mobilize the non-specific immune function of the body and to enhance the immunity of the whole body. If we artificially create a foreign body in the patient as large as possible, wouldn’t it mobilize granulocytes to engulf the foreign body. We chose to scrape on the solar bladder meridian, which has the widest circulation in the body, and checked the patient’s blood picture the next day, and the result was that granulocytes were raised in over 90% of the patients, with the highest being over 120%. This simple and practical gua sha therapy that both improves immune function and well tunes granulocytes to the periphery cannot be dismissed as a model of integration of Chinese and Western medicine. Local targeted immune tumor suppression said: Western medicine first proposed to use biological therapy, systemic immunity, first popular in Japan, life is very short; spread to Taiwan, but also did not see an improvement; spread to mainland China but carried out like a fire ……. The tumor is a foreign body that first releases PDL-1, which causes the programmed death of T cells, not recognized by the surrounding immune cells. How to get the immune system to recognize it, attack it, and then destroy it involves the concept of local immunity. Local immunity, can TCM do that? Of course it can. If we don’t know western medicine and we don’t know the concept of Chinese medicine very well, we can never propose these new theories and methods that are conclusive and effective. Chinese medicine people to quietly read the classics, the language is difficult to understand is by no means the reason we do not accept; the ancients often do not write a clear narrative, often only the two ends of a thing, that is, what and how to do, the middle of the reason is not put forward, read the classics, we must read the “road” to read out, in order to integrate and coherent. (3) must focus on the methodology of understanding research “the concept determines the method, the method determines the results”, this is the unchanging truth, in order to truly integrate Chinese and Western medicine must also be from the methodological efforts. ① from the macroscopic combination of Chinese medicine and Western medicine is a strong combination of the microscopic combination of swelling and pain after a sprain: Western medicine first cold, then hot, cold so that the vasoconstriction, exudate reduction, swelling and pain will be reduced, hot so that the stasis of the site to unblock; but first cold and then hot will generally produce sequelae, is what folk call rheumatic pain, Western medicine according to the pathological form of the sprain from vasoconstriction diastole to treat, is the microscopic understanding. Chinese medicine, I directly use moxibustion, first warm through the blockage, both to reduce swelling, but also to avoid sequelae, Chinese medicine is the external performance of the sprain and take the treatment, Chinese medicine can not see the internal vascular form, that “blood is not good for water”, edema is caused by blood stasis, treatment with moxa warm blood natural edema disappeared, this is macroscopic understanding This is the macroscopic understanding. If in the game field Western medicine treatment can quickly reduce the pain to resume the game, but in other cases with direct moxibustion will be fine, will not leave after-effects. If cold compresses and moxibustion are combined, moxibustion is much stronger than hot compresses and will not leave after-effects, which is the application of strong combination. Western medicine target and Chinese medicine macroscopic identification of targeted drug therapy: Western medicine often selects targeted drugs according to tumor genetic test results, even if the test results are completely in line with the efficiency is 70%-80%, then there are factors other than genes that affect drug efficacy, exactly which genetic factors affect drug efficacy is still unclear, which is the microscopic understanding of modern medicine. We have made a macroscopic understanding of the Chinese medical diagnosis and site identification of drugs such as ERSA and Tricor, and concluded that the indications for Tricor and ERSA are patients with lung adenocarcinoma whose diagnosis is cold and wet, and the lesions are peripheral, and they are mostly combined with pleural fluid and pericardial effusion. Is there any relationship between gene mutation and tumor location? I once asked my student Tianzhen to compile the relationship between the location of lung cancer and the effectiveness of targeted drugs, and found that there is a close relationship between them. Well, if we can identify the indications of targeted drugs with TCM, we can effectively select targeted drugs according to where the tumor grows and what the TCM identification is, even if EGFR is not checked in the clinic. If we can combine TCM identification and target detection, the efficiency of targeted drugs will be greatly improved, which is also an example of strong combination. ② Must focus on complementarity Ⅰ. Combine the multi-target of TCM with the single target of Western medicine: the multi-target of TCM includes a drug with multiple targets and a drug combination with multiple targets, a drug can have multiple effects, and a method can have multiple effects, which is precisely the advantage of TCM. Western medicine is mostly single-target. If the two can be effectively combined, they can complement each other’s shortcomings. II. Complementary advantages: Chinese and Western medicine have their own advantages, so if we use their respective advantages to help each other, the efficacy will be improved naturally. For example, I proposed that “tumor is a xenobiotic organism in which the autologous cells in the region mutate and proliferate after the local internal environment is changed, but is not recognized and inhibited by the surrounding immune cells”, which includes three aspects. This concept has completely expressed the causes of tumor formation. For these three aspects, Western medicine is stronger than anti-cancer. Almost all treatments in Western medicine are mainly aimed at xenobiotics – “tumor”, which is the strong point of modern medicine, but anti-cancer cannot solve the recurrence and metastasis of tumor well. Chinese medicine has advantages in improving internal environment and changing soil; acupuncture has advantages in improving local immune function. Internal environment and local immunity are the most important factors of tumor recurrence and metastasis, which is the strong point of Chinese medicine. If we can combine Chinese medicine to change the internal environment, acupuncture to improve local immune function and Western medicine to suppress tumor organically to achieve strong combination, it will undoubtedly solve all the problems of tumor growth, recurrence and metastasis very well. 4. Combination of Chinese and Western medicine in tumor thinking and practice The following is a few perspectives of our combination of Chinese and Western medicine for your reference, and it is considered as a brick to draw in the jade. (1) Make full use of the anatomy of acupuncture points to use needles We should make good use of the anatomy of acupuncture points to guide the use of needles, which is the basis of our use of needles. Although the essence of meridians is still a mystery, the anatomy of acupuncture points can help us to think about the role of acupuncture points from multiple perspectives, as well as to guide needling and understand the contraindications of needling. You can refer to books such as “Anatomy of Acupuncture Points” by Professor Yan Zhengguo. A few examples are given to give you a sense of understanding. ① Clever use of acupuncture points and the relationship between the surrounding organs: for example, common pharyngitis, we all know that pharyngitis is seen in the formation of follicles in the posterior pharyngeal wall, capillary disorders, local edema, for pharyngitis Chinese medicine on the use of acupuncture acupuncture point Tian Tu, and we believe that the treatment of pharyngitis should stimulate the local, improve the posterior pharyngeal wall edema, for this we according to the anatomy of acupuncture points, Tian Rong point, Lian Quan point is located near the throat, acupuncture Tian Rong point into the posterior pharyngeal wall needle Directly on the local action, and then with Lianquan into the back wall of the pharynx needle also stimulate local treatment of pharyngitis, effective as rafter; and by extension, the pharynx near the disease, such as swelling and pain in the throat after radiation therapy, dry mouth, tonsil tumors, etc., can be taken in the Tianrong and Lianquan points. The ancients had many taboos against needling at the Tien Tu point, and did not dare to enter the needle, and even if they did, it was a shallow stab for fear of life-threatening injuries. Is it that scary? Through the anatomy of the acupuncture points, as long as the correct technique is mastered, there will be no problem even if the needles are stuck deep. And even if the needle is stuck in a blood vessel, there will not be a lot of bleeding, and there will be a sense of fluctuation when the needle is stuck in the blood vessel, just avoid it. This is the wisdom we have gained by making full use of the anatomy of acupuncture points. ②. We once treated a lung cancer patient with loss of appetite, and after using all the methods that usually work well, the patient still did not want to eat. What to do? The stomach is not moving, how to make it move? The stomach is like a long eye to avoid the new needles after entering the needles, and the stomach has to peristaltic movement to avoid them. I asked the student to stab around the patient’s stomach, and in less than a minute, the patient asked for food, and the effect was so fast that we described it as a “second kill”. (3) Using dorsal acupuncture points in relation to their nearby nerves to treat diseases of the internal organs: The dorsal acupuncture points of the bladder meridian correspond to the internal organs and provide great convenience in applying dorsal acupuncture points. Modern research has confirmed that the location of the dorsal points is adjacent to the spinal nerve root, which emits various spinal nerves, and the nerves emitted innervate the sympathetic and parasympathetic nerves of the internal organs of the thoracic and abdominal cavities, as well as certain parts of the trunk nerves. By stimulating the dorsal points, the nerve conduction can treat the corresponding internal organs or body surface disorders. Clinically, we have used blood cupping and fire acupuncture to treat related diseases such as asthma, coronary heart disease, intestinal obstruction, constipation, and gastroparesis with remarkable results. In a patient with lung metastasis from colon cancer, the entire lung was almost invisible on the chest CT, which we described as a “white lung”, and the patient was suffocating and could not lie down all day, sometimes even feeling near death. There are many examples like this. The student, Zhao Weipeng, gave a patient with intestinal obstruction a blood cupping on the corresponding dorsal point, and in less than 2 minutes, the patient was discharged. (2) Using pathophysiology to guide TCM evidence identification All of the students graduated from our college have studied pathophysiology, and it may not occur to you that we can skillfully use pathophysiology to guide TCM evidence identification. ①Use the formation of plaque in the vascular wall is related to lipid deposition, and inhibit the formation of blood vessels by removing phlegm and opening the ligaments: Those who have studied cardiovascular pathophysiology know that the formation of plaque in the vascular wall is related to lipid deposition in the vascular wall and damage to the vascular endothelium, but who can think that we can inhibit the formation of blood vessels by removing phlegm and opening the ligaments. We like to apply ten jujube soup to treat tumors related to phlegm, and after applying it for a period of time, ultrasound found that some patients’ tumors did not shrink but the blood flow inside the tumor disappeared, which precisely provides ideas for inhibiting angiogenesis R&D. Inhibiting angiogenesis does not need to focus only on blood vessels, but can start from inhibiting plaque formation, and the method of transforming phlegm and turbidity and opening up the ligaments and veins as described in Chinese medicine can inhibit angiogenesis. It also shows that the mutual existence of phlegm and stasis is an important tumor pathogenesis. The endometrial lining will change periodically with menstruation. When the endometrium is thickest is when the stagnant blood is heavier before menstruation, and when the stagnant blood is removed after menstruation, the endometrium becomes thinner. This shows that blood stasis exists in endometrial cancer (endometrial thickening). Clinically, triamcinolone acetonide tends to cause endometrial cancer. What conditions and what factors will go downward to cause endometrial cancer? Cold tends to go down and coagulate in the uterus, then it becomes blood stasis. Therefore, it can be concluded that triamcinolone acetonide is a cold medicine, and endometrial cancer is cold clotting and blood stasis. The theory of the sages more than 2000 years ago confirms that our present speculation is correct. (3) Chinese medicine identification by pathological pattern Small cell lung cancer and squamous carcinoma both have the factor of phlegm. In terms of pathological morphology, small cell lung cancer is granular and dense, while squamous carcinoma is more common as striated and scattered. Comparing to nature, those favoring cold like to gather in groups, while those favoring heat are scattered and arranged, then it can be said that fire is more important than phlegm in squamous carcinoma, and phlegm and fire are both important in small cell carcinoma. The nodules formed by pancreatic cancer are raised on the surface of the pancreas, which is analogous to the hard bumps in life, indicating that pancreatic cancer must be heavy on external cold, and at the same time there is internal heat holding phlegm and stasis. (4) According to the location of the tumor on the imaging to help the TCM identification Let’s look at lung cancer first. Lung cancer, whether small cell carcinoma or squamous carcinoma, is related to smoking, so it is on the fire side; those located in the periphery are on the cold side. The scattered nodules in the lungs are related to emotion and will. The CT film of thyroid cancer pulmonary metastasis shows scattered cotton wool-like masses. I deduced from this that patients with scattered lesions in the lungs are mostly related to emotions. By extension, herpes zoster, which looks like a string of pearls and develops rapidly, has the characteristics of wind; irritating cough, paroxysmal, also has the characteristics of wind, with Dai Clam San. Then look at the abdominal wall metastatic tumors: which tumors are prone to abdominal wall metastasis? Ovarian cancer is common, as well as bile duct cancer. Some of the metastatic tumors in the abdominal wall are similar to the manifestations of acute peritonitis. With the idea of treating acute peritonitis, we use Dazhi Chest Tang to treat metastatic peritoneal cancer with remarkable effect, so we can presume that metastatic peritoneal cancer favors phlegm and fire. It can also be speculated that tumors that easily cause peritoneal metastasis also contain phlegm and fire, in fact ovarian cancer and bile duct cancer mostly have phlegm and fire factors. (5) Relationship between site, pathology and yin and yang The site here is different from the imaging, which refers to the part that can be seen by the naked eye. According to Chinese medicine, “clear yang develops in the couples, while cloudy yin goes to the five organs” and “up is yang, down is yin”. In contrast, tumors located in the inner, lower and abdominopelvic cavities are mostly cold, and adenocarcinoma, mucinous and plasma tumors are more common. Take digestive tract tumors as an example, tumors of the oral cavity, pharynx and esophagus are often squamous carcinomas, while tumors below the cardia and up to the sigmoid colon become adenocarcinomas, and tumors of the anal canal become squamous carcinomas again. Adenocarcinoma of the endometrium is common, while tumors of the cervix, which is connected to the outside world, becomes squamous carcinoma. Some parts are not necessarily related to the pathology, but I still classify them according to the theory of Chinese medicine. For example, I artificially classify pancreatic cancer into pancreatic head cancer and pancreatic body tail cancer; pancreatic head cancer tends to have a hot nature of Yang Huang, while pancreatic body tail cancer tends to have a cold nature of pain around large blood vessels. I have always been dissatisfied with the classification of lung cancer by modern medicine, which divides lung cancer into small cell lung cancer and non-small cell lung cancer, while adenocarcinoma and squamous carcinoma are classified as non-small cell, which is not scientific. In my personal opinion, the correct classification is either to separate all three or to group squamous carcinoma and small cell carcinoma together. For the rules of tumor and pathological evolution of various parts of the body, please refer to the book “Ten Lectures on Chinese Medicine Tumor Identification and Treatment by Jin Chang”. (6) Classify chemotherapy drugs and targeted drugs according to their side effects to improve the efficacy. (7) Drug administration is guided by drug side effects. In clinical practice, fever occurs when large amounts of interleukin-2 (IL-2) are used, but it is not easy to determine whether the drug is cold or hot, because the pathogenesis of fever can be cold or hot.
