  1.Surgical wound 1.Shoulder wound: After pressure bandage, the nurse will compress the wound with sandbag for 6 hours after returning to the ward, and after 6 hours, the nurse will remove the sandbag, and after the sandbag is removed, the arm can carry out normal activities, do not put weight on it and do upward movement.  2.Leg wound: after pressure dressing, after returning to the ward, the nurse will compress the wound with sandbag for 6 hours (the right thigh is kept straight, the foot can move lightly), after 6 hours, the nurse will remove the sandbag, after the sandbag is removed, the root of the right thigh continues to be kept straight for 6 hours, if there is no bleeding or hematoma in the surgical wound, the right leg can be moved in a small extent under the bed condition, 24 hours after surgery, if there is no bleeding or hematoma in the wound, the doctor will remove If there is no bleeding and hematoma in the wound 24 hours after surgery, the surgeon will remove the gauze and remove the catheter, and then you can gradually move to the ground and do not bear weight; do not take a bath for 48 hours after surgery.  2. Postoperative diet Easy to digest and light food can be eaten 1 hour after surgery, less soy products and dairy products within 2 days after surgery, no overly hot and hard food within 1 month after surgery.  Third, postoperative medication 1, anticoagulation therapy: the afternoon of the day after surgery or the afternoon of the next day to start taking warfarin, 5-7 days after taking the drug to start testing INR (International Normalized Ratio), according to the INR results to adjust the amount of warfarin, INR optimal target 2.0-3.0, pay attention to the bleeding phenomenon, monitor INR every week; if you have been taking warfarin regularly before surgery, follow the doctor’s instructions; if you take dabigatranate, If you are taking other anticoagulant drugs such as dabigatran, rivaroxaban, etc., take them as prescribed by the doctor; take anticoagulant drugs for 3 months after surgery in general.  2.Acid-suppressing drugs: take acid-suppressing drugs (i.e. stomach protector) for 4 weeks after surgery, 1-2 times a day, one tablet each time, and can be stopped after 4 weeks, specific drugs such as Oxy, Nexium, Polite, etc. are available.  3.Other drugs: take them as prescribed by the doctor.  Post-operative follow-up 1.Regular follow-up after surgery, regular follow-up.  2.When you start to take warfarin, monitor INR weekly, and gradually extend the monitoring time to 10 days to half a month after INR stabilization.  3, about 3 months after surgery, review the routine ECG, 24-hour ambulatory ECG, cardiac ultrasound, etc., according to the postoperative and review situation to decide whether to stop warfarin and other drugs.  4.If you are a foreign patient, you can first improve the examination locally and bring the examination data back to Shanghai for follow-up.
