Cough is a common symptom and can be seen in many diseases. In the treatment of cough, the former people first identified external and internal injuries, and secondly, cold and heat, deficiency and reality. After many years of clinical observation, I have learned that it is difficult to distinguish between external and internal injuries of cough, and they often affect each other or are mutually causative, so the treatment from external and internal injuries seems to be boundless. In contrast, if we start with phlegm, we can first identify the presence or absence of phlegm, then cold phlegm and hot phlegm in the presence of phlegm, and then dryness of the lung and yin deficiency in the absence of phlegm, then we can outline and manage the complexity. The following is a brief description of our experience. Cold phlegm This evidence is mostly seen in patients with recurrent attacks of slow branch. The main manifestations are recurrent coughing attacks, more or less sputum, white or grayish color, or accompanied by chest tightness, vomiting, abdominal fullness, light red tongue, white and greasy coating, and moist and smooth pulse. The treatment is to dry dampness and resolve phlegm, warm the lung and dissolve drinks, using Xiao Qing Long Tang or Ma Heng Er San Tang (Jiao Shu De’s formula) as a remedy. For those with nasal congestion and runny nose and other superficial symptoms, add antiphlogistic drugs as appropriate; for those with shortness of breath and sweating, add Qi-enhancing drugs as appropriate. If antibacterial and anti-inflammatory drugs are used as the guiding principle, it will not be effective. There are many ways to drive away the evil, the emphasis is on the identification, as long as the identification is accurate, and how do you know that ephedra and herbs can not antibacterial! The disease is cured by the evil. The former people have to promote the rule of edema by promoting the lung to promote diuresis, said to lift the pot to uncover the lid. The author draws on this method, the treatment of cough commonly used poria, poria to diuretic promote the promotion of lung, achieved better results. Typical case: A female, 54 years old, coughing and sputum for half a month. She had been suffering from “slow branching” and was obese. This attack was caused by an external infection and was ineffective with various antibiotics. The symptoms include cough, white sputum with high volume, chest tightness and shortness of breath, distention of the abdomen and stomach, loss of appetite, normal urination and defecation, light red and slightly dark tongue, white greasy coating, and smooth pulse. Prescription: Roasted ephedra, almonds, Chen Pi, Poria, Semen, Perilla, fried lycopodium, poria, peach kernel, Su stems, aster each lOg, white mustard seed 6g. After taking 5 doses, the symptoms were greatly reduced and the sputum was less, reduce ephedra to 6g, remove white mustard seed, add white X lOg, take another 5 doses, all symptoms were eliminated. Hot sputum This evidence is mostly seen in people with chronic branch complicating lung infection or acute pneumonia or bronchitis. The main manifestations are cough with coarse breath, yellow and sticky sputum, or swollen and sore throat, chest tightness and chest pain, or body heat and face redness, red tongue with yellowish coating and slippery pulse. The treatment is to clear the lung and resolve phlegm to stop coughing, using Qianjin Reed Stem Soup combined with Scape Drabble and Dazao Diarrhea Lung Soup, and supplemented with diuretics. Because of heat evidence, bamboo leaf and plantain are used instead, and heat-clearing and detoxifying drugs are added as appropriate, but not too much, and the main focus should be on resolving phlegm. For fever, add ground bones and peony bark. Typical case: A male, 31 years old, with cough and phlegm for 5 days. He had fever and vicious chills after getting wet. After taking compound Dazhong Ye tablets and quick-acting cold capsule, he had coughing with yellow sputum in moderate amount, accompanied by chest tightness and chest pain, dry throat and sore throat, red tongue, yellow greasy moss and slippery pulse. On auscultation, the breath sounds of both lungs were rough, and chest X-ray showed that the texture of both lungs was thickened. Prescription: 15g each of rhizome, coix seed, winter melon seed and melon atrophy, lOg each of peach kernel, almond, scape bark, zhebei mother, plantain, scutellaria, 6g of bamboo leaf and 30g of dandelion. after taking 3 doses, the symptoms were significantly reduced, remove winter melon seed and zhebei mother, add 15g of red peony, and after taking 3 more doses, all symptoms were eliminated. Lung dryness This symptom is mostly seen in people with external sensation in autumn, mostly due to autumn dryness injuring the lung. The main manifestations are dry cough without sputum, dry throat, dry nose and mouth, light red tongue with little fluid, thin or white or yellow coating, and small pulse. The treatment is to clear the lung and moisten the dryness to stop the cough. In treating this evidence, attention should be paid to lightly promoting the lung health, avoiding the use of bitter medicine to clear heat, preventing nourishment to moisten dryness, and the amount of medicine used should not be too heavy. Typical case: A female, 33 years old, presented with a cough for 3 days. She has a dry cough without sputum, with dry nose and throat, dry mouth, no fever, normal diet and stool, a light red tongue, white fur and little fluid, and a fine, small pulse. Prescription: 6g each of Mulberry leaf, Chuanbei mother, Tempeh drum and Gardenia, 10g each of Almond, Saxifrage, Medlar, Schisandra, Schisandra and Cicada, 15g of Prince’s ginseng and 1 pear skin. After taking 3 doses of the above formula, the symptoms were obviously reduced, and after taking 2 more doses, the symptoms basically disappeared, and it was advised to eat pear two or three times a day for good luck. Yin deficiency This evidence is mostly seen in those who suffer from slow branch and have Yin deficiency. The main manifestations are dry cough without sputum or a small amount of mucous sputum, dry oropharynx, weakness, or sweating, low fever, red tongue with less coating and less fluid, thin pulse. The treatment is to nourish Yin and moisten the lung to stop the cough, with the help of Qing dryness and save the lung soup or Xue Xue moisten the lung formula (Bei Sha Shen, Yu Zhu, Fu Shen, Dendrobium, sweet almond and raw lentil). In accordance with the theory that long-standing diseases enter the lung, nourishing the blood and moistening the lung, invigorating the blood and opening the lung can be added as appropriate. Typical case: A male, 64 years old, presented with dry cough without sputum for more than 10 days. He has been suffering from “slow branching”, which occurs in winter. He has dry cough without sputum, dry and itchy throat, fatigue, sweating, chest tightness, sleeplessness at night, little appetite, dry stool, dark red tongue, little moss and little fluid, and fine and small pulse. Prescription: 15g each of Radix Salviae Miltiorrhiza, Rhizoma Polygonati, Dendrobium, Almond, Peach kernel, White lentil, Medlar, Fructus Schisandrae, Radix et Rhizoma Ginseng, Cicadelliae each l0g. After 5 doses, the symptoms were significantly reduced and the stool passed, with the removal of Cicadelliae and Radix et Rhizoma Ginseng and the addition of 15g each of Rhizoma Dioscoreae and l0g of Cornu Cervi Pantotrichum. He was advised to take one or two pears daily, and to take Liu Wei Di Huang Wan, two pills daily, to promote positive energy and prevent recurrence. The Sutra on Coughing says, “All the five internal organs and six internal organs cause coughing, not only the lungs.” Most people interpret this verse to mean that cough can be caused by dysfunction of organs other than the lungs, which is certainly true, but from another perspective, this verse clearly reminds us that cough is not limited to the lungs but is not separate from them, and the focus is still on the lungs. As Zhang Jinyue said, “Although there are many coughing symptoms, they are all lung diseases.” The cough, regardless of external or internal injuries, is caused by the lung system being diseased and the lung qi going up. When the lung qi is congested and unable to transfer fluids and fluids, phlegm and turbidity are easily produced, and phlegm and turbidity can block the qi flow and aggravate the cough. Therefore, the treatment of cough can often be summarized by identifying phlegm. Where phlegm is present, it must be dissolved first. If the phlegm is removed, the qi will be cleared and the cough will stop. A common clinical mistake is not to dissolve phlegm when it should be dissolved, or to use dryness when it is necessary to moisten the lung, so that the identification of evidence is not refined and the formula is not cleverly formulated. In addition, it is generally inappropriate to use phlegm-cleansing and yin-nourishing drugs together. Only for a few patients with little phlegm and sticky phlegm, one or two yin-nourishing drugs such as lily of the valley and sage are added to phlegm-cleansing drugs to thin the phlegm and make it easy to discharge. For dry cough without phlegm, it is necessary to nourish Yin and moisten the lung with sweet cold and avoid dry medicine. Nourishing Yin and moistening the lung should also be prevented from nourishing greasy and hindering the stomach. In addition, blood-activating and energizing drugs such as peach kernel and red peony can improve the flow of qi and blood in the lungs and help to expel evil spirits. In addition to the above treatment, care should be taken to avoid spicy and other stimulating foods during the medication period.
