Pain around the shoulder joint is very common, and it used to be classified as “frozen shoulder” due to lack of awareness. There are many different diseases of the shoulder joint, and the location of the pain around the shoulder is valuable in determining the pathology of the shoulder joint. It is important to identify the different diseases that cause anterior shoulder pain. 1. Anterior joint line pain Usually limited, mild, dull pain, (inflammation of the joint capsule, early frozen shoulder, frozen shoulder) – X-ray + MRI. 2. Biceps interosseous groove Usually limited, mild, dull pain, pain when lifting heavy objects – biceps tendonitis – ultrasound. 3, Anterolateral pain Usually diffuse, radiating pain distally, severe pain with overhead motion – rotator cuff lesion (impingement, rotator cuff tear, tendon calcification) 4, Acromioclavicular pain Very specific and limited, dull pain, severe pain with extreme overhead motion, acromioclavicular arthritis – elderly, osteolysis – young – “weight lifting shoulder “. 5, trapezius region pain Continuous dull pain, can radiate, not affected by shoulder joint activity – cervical spondylosis – MRI. 6, rostral synovial pain Rare – limited – pain on internal retraction – rostral impingement, may also be inflammation of the joint capsule, secondary manifestation of arthrosis. 7, sternoclavicular joint pain limited, lighter, inversion, increased pain with overhead motion – sternoclavicular arthritis.
