How does macular cleavage develop?

A macular cleft is a split between the anatomical layers of the retina in the macula. The split is not longitudinal across the entire retina, but is a split along one of the layers of the retina. Macular cleavage occurs very slowly, and patients can have years of slow vision loss under constant vitreous traction with little notice. The lesion is extremely subtle and the presence of macular cleavage can only be detected on an OCT scan. Some patients can be treated with biosection + inner boundary membrane tearing surgery, the actual surgical procedure is more complicated and. The results of the surgery are satisfactory in some patients.
