Cough is a very common symptom. A so-called chronic cough is one that is still present for more than two months and is especially common in women. It is important to pay attention to this chronic cough that is asymptomatic on chest X-ray. Many such patients go to the hospital and use many antibiotics without solving the problem. I started to pay attention to this problem ten years ago when a female cadre from the Shenzhen Procuratorate, who was coughing as soon as the court was in session, was treated according to chronic bronchitis, and the treatment did not work for a year. After examination, we felt that this patient did not look like a chronic bronchitis and decided that it was asthma. After treatment, she got better. However, she would still cough when she encountered cold air. We did a bronchoscopy on her and found no major problems. Sometimes we discussed with her and she said that she often felt bloated and full, and that her cough was aggravated by her stomach. Later we found out after examination that her cough was caused by gastroesophageal reflux, so we treated her stomach and her symptoms improved greatly. Then she went to a hospital in the south and had a gastroscopy to find a H. pylori infection, and after using some symptomatic medications, her cough cleared up. This patient had a cough caused by gastroesophageal reflux and at the same time was asthmatic in the form of a cough. I cite this example to show that cough is not necessarily purely an inflammation of the airways. The causes of cough are complex and can be respiratory, gastrointestinal, or otorhinolaryngologic diseases. I had a child patient who was always coughing, coughing twice each time, but no one could find the cause. The Nei Jing says that “all five internal organs and six internal organs cause coughing, not just the lungs”. The actual fact is that you will be able to get a lot more than just a few of the most popular and most popular items. The actual cough is often caused by rhinitis and sinus lesions. A patient with allergic rhinitis or sinusitis is sometimes not reflected in the nose, but manifests itself in the form of a cough. These pictures show the postnasal drip phenomenon, glandular hyperplasia, and cobblestone-like hyperplasia in the posterior wall of the throat, all of which can cause a cough. This child coughs a lot and his nose is always tickling like hell. This is a typical cough caused by allergic rhinitis, and once the rhinitis is controlled, he stops coughing. The second cause of coughing is asthma. We all know about common asthma, but some patients don’t wheeze, they just cough. I have a patient who is an engineer and he coughs at 3 o’clock at night. With this one you can tell that this patient is asthmatic. We have children who run and cough in the morning and also cough a few times in the evening, in this case we need to pay attention to see if this child has asthma. If the symptoms are right and treated as asthma, the cough may be cured quickly. The third cause of cough is eosinophilic bronchitis. We have patients who cannot be detected by any tests. We use ultrasound nebulization to guide some sputum for sputum cytology, in which eosinophils exceed 3%, and this patient can be diagnosed with eosinophilic bronchitis. Antibiotics are useless in this type of patient, but glucocorticoid treatment will be effective. The fourth type of chronic cough is caused by stomach problems. I had an 18-year-old patient who went to Huazhong University of Science and Technology from Guangxi and always had a cough. The patient said he often felt uncomfortable with his stomach, belching, bloating and acid reflux, so I noticed this problem and later found out that his cough was actually caused by gastroesophageal reflux. We did acidity monitoring on this patient, with two very thin electrodes, and with esophageal reflux, the acidity increased and the PH decreased. If there is a problem with pneumography is another thing, but if there is a cough with pneumography problems, it may not be a problem with the trachea and lungs. Instead of taking antibiotics all day long, go to the doctor and look for the cause in other ways. Also there are some blood pressure lowering drugs (angiotensin converting enzyme inhibitors) and 20% of people in China take these drugs and get a cough. We have a radiology chief with this kind of symptom, a cough caused by taking blood pressure lowering drugs.
