This has led to the emergence of the small right-sided incision. The reason why minimally invasive is in quotation marks is that only the position of the incision is changed, the heart operation is not small at all, even the size of the incision is sometimes not small, but most of them are smaller compared to the median incision. The right lateral incision has several advantages compared with the median incision: 1. The incision is on the right lateral side, the scar is hidden and the anterior chest incision scar is avoided to achieve an aesthetic effect; 2. The operation enters the chest from the intercostal area without sawing the sternum. Small trauma, less blood leakage, less blood loss, no fixed wire left behind, ensuring the integrity of the thorax, no postoperative chicken chest; 3, for some surgeries, the surgical field is more satisfactorily revealed, and the right atrium and interatrial septum are directly in front of the field of view, easy to operate, unlike the right atrium in the side of the median incision, when the ventricle is enlarged, the right atrium is in a deeper position and difficult to operate; 4, if the median incision is made, in the future, due to other diseases need to operate again, adhesions The right lateral incision reduces this risk. Are there any disadvantages to the right lateral incision? It has been reported that the right side incision will have problems such as scoliosis and asymmetric breast development, but during the actual work, we found that our right side incision is far away from the location of breast development and closes the rib muscles reasonably, so those complications will not occur, which of course should be the result of decades of improvement by Director Liu Yinglong. The right side incision is so good, then why is it carried out in few places other than Anzhen Hospital, which advocates doing the right side incision? 1, the heart is located on the left side of the chest, right in the middle of the open chest, the heart is behind the sternum, if the right side of the open chest, the heart location will be very deep, as the child gets bigger the position is deeper, the more difficult the operation. 2, the right center is located on the left side of the operation, in the right side of the incision becomes the back, so the operation is very difficult. So in addition to Director Liu’s method, the operational requirements are also very high, which invariably increases the risk of surgery. It is not particularly experienced and very dangerous. The right side incision is mainly used for simple congenital heart disease such as atrial septal defect and ventricular septal defect, sometimes also do combined right ventricular outflow tract stenosis, arteriovenous catheter failure perpetual left superior vena cava and other malformations, and even surgery with more back operations such as Fallot’s tetralogy. Personally, I have a lot of experience with right-sided incisions, and I will take the initiative to suggest right-sided incision surgery, but I am not too willing to initiate surgery with combined back operations, because the risk is too great and too tiring, unless the family strongly requests it.
