I. Initial consultation
The patient, a 32-year-old female, described that she had frequent acne and pimples on her face before she was 30 years old, and went through many treatment programs. The purpose of this visit to the clinic was to remove these depressed scars. On examination, the patient was found to have densely arranged depressed scars on the left side of the face, and longitudinally arranged persistent red spots and scattered pigmented spots could be seen on the front side of the ear screen. Since the patient’s symptoms are clear and the onset of the disease is clear, the diagnosis of depressed scars can be made clearly without auxiliary examination.
II. Treatment process
The depressed scars on the patient’s face had lasted for more than 2 years. For this treatment, the patient excluded topical medications and gold RF microneedling and finally chose the relatively cost-effective CO2 fractional laser. Lidocaine hydrochloride gel was applied to the patient’s face for local anesthesia, and after 30 minutes, the anesthetic was removed and the local iodophor solution was disinfected. The patient was instructed not to get wet for 3 days and to have a second treatment at an interval of 60 days, and the patient left the office after expressing his clarity.
III. Treatment effect
The patient returned to the clinic after 60 days, and it was visible to the naked eye that the depressed scars on the face had improved significantly, the persistent redness in front of the ear screen had gradually faded, the hyperpigmentation spots had also become significantly lighter, the scattered larger pores on both sides of the nose had gradually shrunk, the skin surface looked more complete, and the smoothness of the surface had increased when touched by hand. The patient said that the scab started to form on the face about 1 day after the laser treatment, and started to fall off gradually about 3 days, and the scab disappeared completely 7 days after the treatment, and the depressed scars on the face gradually became lighter and the red spots gradually faded within 60 days.
IV. Notes
We are glad that the patient’s depressed facial scars have gradually lightened after the CO2 fractional laser treatment. After laser treatment, scabs will appear on the face. Do not tear off the scabs by hand before they fall off completely on their own, otherwise it may lead to significant local skin breakage. Do not drink alcohol for 3-7 days of treatment, do not eat too much spicy and stimulating food, and reduce sweating to avoid stimulating local tissues and triggering local co-infection with bacteria. In the process of scab recovery, if the local skin appears red, swollen, painful, consider bacterial infection, it is recommended to consult a doctor in a timely manner and use antibiotic drugs for treatment to bring the symptoms under control as soon as possible.
V. Personal insight
1, depressed scars itself is a kind of scar left after the face acne is stimulated by external force and squeezed and broken.
2, depressed scars may gradually become lighter within a few years for those with shallow scars, and if they are deeper, they will need corresponding physical treatment to recover.
3, for the treatment of depressed scars, silicone gel can be applied appropriately to those who have just formed, and carbon dioxide fractional laser or gold RF microneedle can be chosen for old depressed scars.
4. Although there will be obvious changes in depressed scars after treatment, it requires a full course and periodical treatment to fully restore the symptoms of depressed scars to a healthy state.
