Celiac disease was once a so-called “disease” that plagued many women, and when they went for a medical checkup, almost nine out of ten would be diagnosed as celiac disease. A. Celiac disease is actually a misconception of a normal manifestation of the cervix in the past. Why has celiac disease been wrongly perceived as an abnormal condition in the past? If you have a gynecological exam, what your doctor can see from inside the vagina is that yellow part in Figure 1, which is the appearance of the cervix. On the part of the cervix, there are two different types of cells, as shown in Figure 2, the squamous epithelial cells near the inside of the vagina and the columnar epithelial cells near that direction of the uterus. The two types of epithelium behave differently in appearance. The appearance of the cervix as seen under gynecological examination. The central part of the cervix, which looks a bit like “erosion”, is covered by columnar epithelium, while the outer part of the cervix, which is relatively smooth, is covered by squamous epithelium. This area is medically known as the “squamocolumnar junction zone”, which is also a favored area for cervical cancer (cervical cancer and cervical erosion are not necessarily correlated, as explained below). The squamocolumnar junction zone is susceptible to estrogenic influences. Before puberty, when ovarian function is not perfect and estrogen is low, the columnar epithelium is more medial. After menstruation, the columnar epithelium is influenced by estrogen and develops more towards the outside, so that more columnar epithelium like “erosion” appears at the cervical opening and is detected during the examination. After menopause, when women’s estrogen levels drop, the columnar epithelium begins to retreat back into the cervical canal, so by this time the “vesicles” are no longer visible on examination. So, in essence, what is called cervical erosion is actually an ectropion of the columnar epithelium, a normal physiological phenomenon, not a pathological disease. In the past medical textbooks, there was a so-called graded diagnosis of cervical erosion, called mild, moderate and severe, where the size of the extent was considered to be the degree of inflammation, with an area of less than 1/3 being mild, 1/3 to 2/3 being moderate and more than 2/3 being severe. If you understand the real mechanism of the so-called “cervical erosion” mentioned earlier in this article, you will understand very well that this is actually the different degree of columnar epithelial ectropion affected by estrogen, which is a normal physiological phenomenon. It is a normal physiological phenomenon, regardless of whether it is mild, moderate or severe. Second, what are the clinical manifestations? Celiac disease is a physiological phenomenon, nothing special performance, there may be contact bleeding performance, but this is only the individual differences of the cervix, just like some people chew something hard, a little blood in the mouth, is understandable. Here it is necessary to mention cervicitis, if there is an increase in leucorrhea, yellowing and odor, these are the manifestations of cervical inflammation, which are the symptoms that appear after the infection has appeared on the cervix. The cervical cyst and hypertrophy are also the result of chronic inflammation of the cervix. Third, should I treat it? “Celiac disease” is a normal physiological phenomenon and does not require any treatment. Nowadays, if you look up the many treatments for celiac disease on the Internet, they are all wrong. By the way, for symptomatic cervicitis, treatment is required. The specific treatment method needs to be determined by different hospitals, but usually, acute inflammation can be treated with suppository medication, while chronic inflammation can be treated with physical therapy methods such as laser or freezing. Do I need regular checkups? Regular cervical checkups are necessary, not to prevent “celiac disease”, but to prevent cervical cancer. V. Will cancer develop if left untreated? Cervical cancer is associated with human papillomavirus (HPV) infection, and some high-risk HPV types, when persistently infected in the squamous junction of the cervix, are prone to precancerous lesions and cervical cancer. Since the availability of cervical smear screening, the mortality rate of cervical cancer has decreased dramatically, and the key is early prevention and treatment. Currently, it is recommended that women after the age of 21 should undergo cervical smear screening once a year. After the age of 30, HPV screening can be combined with HPV screening, and if three consecutive HPV and cervical smear tests are negative, the interval can be extended to once every three years, and screening can be stopped after the age of 65. VI. Does it have an effect on fertility? Understanding that celiac disease is a physiological phenomenon means that it will not have an impact on fertility. Why are so many hospitals still treating celiac disease? The change in the concept of “celiac disease” in China was officially written into the textbook after 2008, but many doctors still do not understand and learn this new concept and are still diagnosing and treating celiac disease, which is actually a wrong treatment.
