Focus on occult penis

The Department of Male Medicine of Wenling City Hospital of Traditional Chinese Medicine has successfully cured another case of a child with occult penis. The child’s penis, which was only 0.5 cm long before surgery and looked like a small mound, regained its normal appearance, and its natural length grew to 2.7 cm. A week later, the child was happily discharged from the hospital. This is the 151st case of occult penis cured in the department. 10-year-old XiaoXinYuan, who lives in Shitang Town, Wenling City, was born soon after his parents noticed that although his head was growing fast, his external genitalia were almost as if they had never grown. At first, the parents thought it was due to stunted growth, so they paid special attention to his nutrition, but no matter how to supplement nutrition, his external genitalia is not growing, but rather the surrounding skin is getting thicker and thicker, piling up like a small meat flab, every time you pee will get your pants wet. After a careful examination, it was found that the child’s penis was short in appearance and was in the shape of a small mound, but when the affected area was touched, the normal penile corpus cavernosum could still be reached in the skin of the pubic bone (root of the penis). He was diagnosed as having a rare “occult penis”, and on July 5, he underwent “occult penis correction penile lengthening surgery”. The operation first cut the skin on the dorsal side of the child’s penis, pulled out the corpus cavernosum, then removed the stunted meatus (the tissue between the skin and the penis), and finally repositioned the penile skin and sutured it. According to the introduction, “occult penis” is relatively rare, the incidence is only about one in three, and even professional textbooks are less involved. The root of the disease is easily confused with genital dysplasia and other diseases. Parents should be concerned about their children’s sexual development, and if they find that their children have a short penis, they should take them to a urological male specialist for examination. If not treated correctly and in a timely manner, it will aggravate the deformity, affect the function and cause great psychological pressure on the child. The main pathological features are: (1) the penis is hidden under the skin and the fibrous cords restrict the normal extension of the penis; (2) there is too much subcutaneous fat in front of the pubic bone; (3) the penile skin is poorly attached to the penile corpus cavernosum, resulting in the corpus cavernosum being hidden in the subcutaneous tissue in front of the pubic bone; (4) circumcision has been performed and in a few patients, the ring-like cords caused by scarring cause the penis to fall into the subcutaneous tissue; (5) prepuce prevents the penis from extending normally. Surgery is the only way to treat occult penis, specifically the Devine method, Maizels method, Shiraki method and other methods. At present, our hospital combines the advantages of various surgical options and adopts a double Z-shaped incision on the pubic bone or an arc-shaped incision, removes part of the subcutaneous fat, decapsulates the penis, and dissociates the fibrous cords around the penis and the superficial suspensory ligament of the penis, so that the penis can be fully stretched and thus the penis can be lengthened.
