Talking to Oncology Patients about Recovery

Ya Jun, since I learned that you are suffering from tumor, I have been worried about your condition, and my heart has never been quiet, my mood is changing all the time with your condition: if your condition is good, my mood is good; if your condition is bad, my mood is bad. I’ve always wanted to say something to you, to see you, but I haven’t found a suitable opportunity. The day before yesterday was the day you underwent surgery, and I was too far away to visit you. Taking the opportunity of vacation, as a friend and as a doctor, I would like to talk to you about the recovery of tumor. I also hope that these contents will be helpful to all friends with tumor patients. Shi Hanping, Department of General Surgery, Aviation General Hospital of China Medical University First of all, don’t blame yourself. Ya Jun, having tumor is not your own fault, nor is it because you are not kind, nor is it because you have done something bad or your seniors have done something bad. Many tumor patients, especially young tumor patients, almost always ask the same question: “I am so kind, why do I have tumor?” Such a thought will only bring three psychological results: one is indignation, thinking that God is unfair to them and the earth is unfair to them; the second is infinite self-blame, thinking that they or their family members must have done something evil. The third is extreme remorse, “I should have known better, I should have been more kind, more helpful, more ……”. In fact, the cause of tumor is still not identified, only that it is the result of the combination of environmental factors, lifestyle and genetic factors. There is no relationship between whether a person gets sick or not and the goodness of human nature, and it is not true that good people do not get sick, nor is it true that evil people must get sick. Remember the old saying, “Good people don’t live long, but bastards live a thousand years”? The good man is not a nationally recognized good man? Why did he also die at a young age? Second, learn to be optimistic. It is indeed very, very, very unfortunate that you are so kind, so warm-hearted, so optimistic, so sunny, yet you are haunted by the tumor demon. But, Ya Jun, please believe that you are not the most unfortunate. Remember the little girl in the blood ward, whose parents died at the age of 3, who discovered leukemia at the age of 8. Despite countless chemotherapy treatments, she graduated from elementary school with the top overall score in the school and was guaranteed entry into junior high school; remember the ZWF I told you about, who is only 24 years old, smart and beautiful, from a wealthy family, who has already had her 4th surgery this year, and whose tumor has now metastasized; remember ” If you give me three days of light” of Helen Keller? They are much more unfortunate than you. If you think about those who are more unfortunate than yourself, you will have more confidence and will strengthen your courage to live. Some people say that “one third of tumor patients are scared to death”, which is very reasonable. In fact, tumor is not as scary as imagined, and there are many stories of cancer fighting stars. It is best to face it frankly. The glory of life is not how long you live, but how wonderful you are. None of the people who are famous for centuries are because of their longevity, but because of their contribution. The treatment of tumor depends both on doctors and on oneself; the treatment of tumor depends both on drugs and on will. Thirdly, timely treatment. I know that surgery, radiotherapy and chemotherapy are all tougher, but they are the best way to treat tumors at present. In order to avoid long pain, you may as well endure short pain. In fact, tumor surgery is almost no different from normal surgery, it is not more traumatic and has no more negative impact on human body. Radiotherapy, chemotherapy do have some side effects, but they are not unbearable. Although some patients failed to persist, more patients completed the treatment. The side effects of chemotherapy usually appear within a few hours after treatment and disappear in about 5 days, appearing fast and fading fast; the side effects of radiotherapy appear later and last longer. For tumor treatment, there are two kinds of mentality to be avoided: one is too light-hearted: you think you are fine now, you can eat and drink, you can move freely, unlike patients, it does not matter if you do not treat. In this regard, please look farther ahead and look at the future longer; the absence of things now does not mean that the tumor does not exist, but there is a development process, and if you wait until many manifestations appear before treatment, it may be too late. Secondly, too much importance is attached to it: thinking that the bigger the surgery is, the better the cut is; thinking that the longer the chemotherapy/radiotherapy is, the less the tumor cells will be, so take the initiative to ask the doctor to increase the course of treatment and increase the dose; both of these mentality are not conducive to the recovery of the disease. It is better to listen to the opinions of different hospitals and experts before making choices and decisions. Fourth, reintegration into society. One of the important elements of tumor treatment is psychotherapy. Reintegration, that is, returning to the work and life before treatment is an important psychological treatment. Many patients stop working and rest at home after the discovery of tumor, and their family members ask their loved ones to quit their jobs and go home out of concern for them, which results in the opposite, but is detrimental to patients. One important purpose of going back to normal life and work is to divert patients’ self-tumor attention through normal work and life, avoid negative introjection and forget that they are tumor patients. In 1998, I did a rectal cancer resection and liver metastasis resection for a patient with advanced rectal cancer. the patient was a dance lover, and she kept on her interest and dancing after the surgery. it has been 14 years now and there is no problem in the review. However, return to work should pay attention to the combination of work and rest, too heavy physical labor, too much work pressure, too strong occupational challenge is also a disadvantage for tumor patients. Fifth, follow up on time. The major difference between benign and malignant diseases is that malignant diseases will recur (resurface in the same place) and metastasize (new growth in a different place) after treatment, while benign diseases do not have this phenomenon. Therefore, after malignant tumor treatment (including various treatments), it is necessary to review regularly in order to detect recurrence and metastasis at an early stage for timely treatment. It is found that 80% of tumor recurrence and metastasis after surgery occurs within 3 years after surgery, 15% occurs within 4-5 years and 5% occurs after 5 years. Therefore, medical science generally requires: in the first 3 years after surgery, review once every 3 months; in 4-5 years, review once every 6 months; after 5 years, review once every 12 months. Because distant metastasis after tumor surgery is generally more than local recurrence, so in the review, we should focus on the most frequently metastasized parts, such as liver and lung after gastrointestinal tumor surgery is the key examination site; when tumor metastasis and recurrence, biochemical parameters such as tumor markers are generally elevated first and morphological changes are later. Generally, morphological changes will appear only after 3-6 months of tumor marker elevation, so it is important to have an objective attitude towards the negative results of morphological examination. Weight is an important parameter that must be observed at each follow-up visit. Progressive weight loss is a bad prognosis. Sixth, strengthen nutrition. Nutritional support for tumor patients includes daily diet at home and professional nutritional treatment in hospital. It is found that one-third of tumors are related to daily diet, and the relationship between digestive tract tumors and diet is even closer. In daily life, we should pay attention to (1) reduce calorie intake: seven, eight minutes full is good, as the saying goes, eat one or two bites less per meal, live easily to 99. (2) reduce fat intake: animal meat contains high-quality protein, better than plant protein, we can not not eat meat; but we should eat less red meat, less processed meat, more white meat; recommended white meat 2-4 times a week, each time 50-100g (1-2 two). Vegetable oil is better than animal oil, perennial plants such as tea oil, olive oil is better than annual plants such as corn oil, vegetable oil; (3) increase fruits and vegetables: fruits and vegetables are rich in vitamins, minerals, antioxidants, the American Cancer Society recommends 5 servings of fruits and vegetables daily (equivalent to 5 fruits, such as an apple, an orange, a banana, etc., or 500g of vegetables). (4) Increase grains: Grains include (large, small, black, oats) wheat, rice, corn, sorghum, millet, etc.. They are rich in vitamins, antioxidants and a variety of antitumor substances such as phenolic acid, tocopherol, inositol 6 phosphate, lignans, etc.. Because the micronutrients of grains in the processing process will be seriously damaged, so grains should not be refined, advocate food is not averse to coarse, grain is not averse to miscellaneous. Seventh, moderate exercise. Exercise can help tumor patients in many aspects, including reducing excessive body weight, improving metabolic status, enhancing immune function, preventing and controlling muscle loss, reducing toxic side effects of treatment, improving quality of life and prolonging survival time, etc. The effect on breast cancer, colorectal cancer and prostate cancer is even more significant. 2010 American College of Sports Medicine recommended tumor patients to take at least 5 times of moderate to vigorous exercise per week, 30-60 minutes each time. -60 minutes. However, depending on the patient’s physical status and tumor stage, at least one moderate exercise session of 30 minutes or more per week is the minimum requirement. Basic daily physical activity is not a substitute for physical activity. Yajun, I wonder if you have sports facilities at home or in your neighborhood? If not, the easiest but very effective exercise is to take a brisk walk after dinner. The most important meal for us Chinese is dinner, the best and most eaten, so choose after dinner. The basic requirements of walking after dinner are two: one is fast, ordinary walking is not useful, or not very useful; second is long, the time can not be shorter than 30 minutes, otherwise the effect can not be achieved. The mechanism of the role of brisk walking after dinner includes (1) increase energy consumption, directly reflected in sweating; (2) promote the gastrointestinal tract movement, the best performance is defecation after exercise. Eighth, change habits. Tumor is a kind of lifestyle-related disease, which can be prevented by changing lifestyle habits. Tobacco is harmful to tumor patients and should be strictly controlled. Alcohol consumption is closely related to the increased incidence of kidney cancer, liver cancer, breast cancer, head and neck tumors and other tumors, and has a direct negative effect on the prognosis of head and neck tumor patients, so head and neck tumor patients must stop drinking alcohol. A small amount of alcohol can help improve cardiovascular disease. For tumor patients with severe cardiovascular disease, a small amount of alcohol can be allowed, no more than 2 servings per day for men and no more than 1 serving per day for women. Here 1 portion is 1drink, according to the U.S. NIH standard 1drink is equal to 18g of alcohol, equivalent to 1 pot or a small bottle of beer 355ml (5% alcohol), red wine 150ml (12% alcohol), imported foreign wine 45ml (40% alcohol), 53 degree white wine 33ml (equivalent to 1 two); tumor patients without cardiovascular disease is best not to drink alcohol. Green tea is an internationally recognized health drink, in which the tea polyphenols have multifaceted health effects, such as anti-inflammatory, antioxidant, improve metabolism, etc., specific important protective effect for tumor patients. As green tea tea polyphenols have the effect of stimulating gastric acid secretion, drinking tea on an empty stomach may cause stomach pain, so it is recommended to drink tea after meals; tea polyphenols also have an excitatory effect, drinking tea at night may affect sleep, so it is recommended to drink tea during the day. However, with the increase in tea, the extension of time, the body can gradually adapt to or tolerate tea polyphenols, and over time, drinking tea on an empty stomach, tea at night on the human body are no adverse effects.
