The lumbar spinal canal is a bony canal. Abnormalities in the structure of the spinal canal due to primary or secondary factors, narrowing of the lumen of the spinal canal, and the appearance of low back pain with intermittent claudication as the main feature, is known as lumbar spinal stenosis. It can be divided into primary (congenital) and secondary (acquired) forms. Secondary degenerative lumbar spinal stenosis (DLSS) is the most common. As people live longer, the aging of the body and chronic strain lead to degeneration, hyperplasia, hypertrophy and disc herniation of the lumbar spine and its affiliated ligaments and joint capsule, resulting in narrowing of the lumbar spinal canal, lateral saphenous fossa and intervertebral foramen, which provokes or compresses the nerve roots and causes pain and dysfunction, and Mr. Yu belongs to this condition. First of all, do not be afraid of having lumbar spinal stenosis, rest is the best treatment, and bed rest is needed for 2 – 4 weeks. Massage is not advisable, especially inappropriate massage, so as not to pull and stimulate to aggravate inflammation and edema. We mostly use a combination of Chinese and Western medicine first. To improve the blood circulation of the combined tissues inside and outside the spinal canal, especially to enhance the blood supply to the nerve tissues, improve the ischemia and hypoxia of the nerve roots and cauda equina in the spinal canal, eliminate or reduce the inflammatory or compressive edema of the soft tissues protruding into the spinal canal, and strengthen the stability of the tissues around the spinal canal. The purpose of surgery for lumbar spinal stenosis is to relieve the mechanical compression received by the nerve tissue, but in some patients the long-term postoperative results are not as good as they should be. This is because surgery itself is also traumatic and may destabilize the vertebral body, as well as cause aseptic inflammation, leading to recurrence of symptoms. Therefore, it has been argued that surgery must be chosen with care. Lumbar spinal stenosis is a progressively aggravating disease, and if conservative treatment is ineffective and the stenosis is indeed bony, surgical decompression is the only option.
