What should I do about intraductal carcinoma of the breast with early invasion?

  Patient: Hi, I have intraductal carcinoma with early invasion ER++ PR- HER-2 (-) E-CADHERIN (+) K903 (-) SMA, P63 partially positive lymphatic clearance 0/7. The mass is 2.0*1.0 in the outer lower limit near the stomach with smooth quality nipples without overflowing activity. I am currently doing chemotherapy for CAF once and am going to do it a second time, and I am also taking Tamoxifen and Paxil tablets. Do I still need to do radiotherapy?  Doctor: I wonder if your pathology report shows any positive post-operative lymph nodes? If the axillary lymph nodes are negative, the NCCN 2008 guidelines call for radiation therapy to the whole breast with tumor bed push irradiation (using photon irradiation, brachytherapy, or electron beam). If chemotherapy is indicated, CMF chemotherapy (class 2B) or sequential chemotherapy may be given concurrently with radiotherapy. If the axillary lymph nodes are positive, then the scope of radiotherapy has to be increased. For now, chemotherapy and endocrine therapy should be given first. Doctor: Tamoxifen tablets are the endocrine therapy drugs, which are very critical drugs. And Pingxiao tablets are not.  Patient: Thank you, Professor Zheng. I would also like to ask you about triamcinolone acetonide and tamoxifen tablets, are they both for endocrine treatment? Which one has less side effects? Which one is more suitable for me to take?
