A 32-year-old woman with a diagnosed ovarian follicular cyst healed spontaneously after dynamic observation

Abstract: A 32-year-old woman, Ms. Wang, was found to have an ovarian cyst at an outside hospital and presented for surgery on this occasion. Ovarian follicular cysts are one of the common female benign tumors, usually occurring in sexually active fertile women, formed by unabsorbed follicular fluid, and are clinically asymptomatic in the vast majority of cases. After admission, Ms. Wang’s ovarian follicular cyst disappeared after clinical dynamic observation and re-examination of ultrasound, achieving clinical healing.

I. Initial consultation
In July 2019, Ms. Wang came to the hospital with the discovery of ovarian cysts requiring surgical treatment. Upon inquiry, her last menstrual period was on July 9, 2019, with a 5-day clean period. She usually has regular menstrual periods, had an ovarian cyst found during a physical examination, and now has a 3-day clean period and requires surgical treatment. General examination revealed a body temperature of 36°C, pulse rate of 82 beats/min, respiration of 20 breaths/min, blood pressure of 100/70 mmHg, normal development, decent features, no abnormal heart and lung auscultation, liver and spleen were not palpated, abdomen was flat and soft, no pressure pain or rebound pain throughout the abdomen. Gynecologic examination showed normal vulvar development, married transitional style, vaginal patency, cervical erosion of degree I, normal size of uterus, no abnormality in left adnexal area, right ovary enlarged about 6cm×6cm×6cm, smooth surface with normal activity, no obvious pressure pain. Vaginal ultrasound examination showed no abnormality in the uterine cervix, no abnormality in the left adnexal area, and a 6cm×7cm×6cm cystic mass in the right ovary with thin wall, good translucency and smooth surface. Other auxiliary examinations showed a negative urine pregnancy test and normal gynecologic tumor markers.
II. Treatment history
According to the comprehensive analysis of ultrasound, gynecological examination, gynecological tumor markers and medical history, the cyst was considered to be ovarian follicular cyst and was recommended to go home for dynamic observation. 1 month later, Ms. Wang had another ultrasound examination and found that the follicular cyst had shrunk significantly. 1.5 months later, Ms. Wang had another ultrasound examination 2 days after her menstruation and the cyst had completely disappeared. Therefore, it was proved that the cyst was an ovarian follicular cyst and did not require surgery and oral medication, and usually disappeared on its own.
Treatment effect
After two ultrasound examinations, Ms. Wang’s cyst had shrunk significantly, and the cyst had completely disappeared by the second examination. This type of cyst will disappear on its own without medication, thus avoiding the pain associated with surgery. This shows that regular review is more important for comprehensive analysis of localized ovarian cysts or masses.
IV. Notes
We are glad that Ms. Wang’s ovarian follicular cyst disappeared after dynamic observation. However, after the ovarian follicular cysts have completely disappeared, Ms. Wang is advised to have regular gynecological checkups to avoid the reappearance of ovarian follicular cysts. If large cysts appear, they may require clinical surgery if they cause torsion, so it is recommended to have an ultrasound checkup once a year, and for small cysts, blind surgery is not recommended, but a review after menstruation. Also in daily life, it is recommended to keep a happy mood and build confidence to overcome the disease.
V. Personal insight
Ms. Wang came to the hospital for surgical treatment after her menstruation cleared because of the high mental stress caused by her first visit to an outside hospital. After a series of clinical examination and analysis, it was found that ovarian follicular cysts were very likely, and after observation, the cysts disappeared, avoiding the harm caused by surgery to Ms. Wang. In the case of ovarian follicular cysts, the observation period of 2 menstrual cycles is very important.
