  The hematoma caused by the surgical operation injury or bleeding may stimulate the body to produce fibers, wrap the implant and contracture, causing the deformation of the implant and the change of breast shape, then the implant needs to be removed.
  Case 2
  Miss Z is a slightly fat married woman who has no plans to have children or breastfeed again in the future, and her mammary glands have not sagged significantly despite atrophy.
  Surgery Plan
  I told her that the two commonly used surgical options for breast augmentation were silicone gel implants and autologous fat injection. After introducing her to both procedures, Ms. Z chose the autologous fat injection for her breast augmentation.
  Advantages
  The biggest advantage of autologous fat injection is that it can kill two birds with one stone, as it completes the sculpting of body curves and at the same time transplants the extracted fat to increase breast volume. There is a poem: we don’t produce beautiful breasts, we are just fat carriers.
  After fat injection breast augmentation, the feel is softer and closer to the natural breast.
  Disadvantages
  The survival rate of fat grafting varies greatly among individuals and can fluctuate between 30% and 70%, some beauty lovers need multiple injections to achieve the desired results.

  On palpation and ultrasound, the mammary glands were found to be almost as small as those of an undeveloped child. Such a woman with very small mammary glands would definitely feel hard with silicone gel implants, so she wanted fat injection breast augmentation to ensure the feel.
  According to our clinical experience and relevant laboratory research, if the mammary glands themselves are small, the local pressure after fat injection is not conducive to the survival of the fat. Besides, Miss F was very thin and did not have enough fat sources.
  Surgical plan
  We recommended a combined negative pressure suction-fat injection approach.
  The first step was to use a negative pressure breast aspirator (BRAVA) for 3 months. At the same time, she increased her weight and fat cell volume by eating more meals and high-calorie foods. After 3 months of suction, the local skin of the breast was loosened a bit.
  For the first fat aspiration we obtained nearly 300 mL of fat from both thighs and injected it into both breasts. Post-operatively, we further promoted fat viability with BRAVA suction, and three months post-operative MRI showed almost half of the fat viability.
  Three months later, 200 mL of fat was obtained from the lower abdomen, and for the second injection into the breast we corrected the asymmetry in bilateral breast size and continued BRAVA suction. After a 3-month recovery period, Ms. F finally achieved a relatively satisfactory upper circumference.
  Benefits
  The combined method of negative pressure suction and fat injection increased the survival rate of the fat and ensured the feel of the breast after augmentation.
  Since the breasts themselves are relatively larger, they can be further upgraded by implants in the future, while avoiding a hard feel.
  Disadvantages
  Cannot maintain fat volume if high calorie intake cannot be maintained after implantation. If you lose weight in the future, your breasts may also “lose weight”.
