Abstract: A 40-year-old female patient was found to have a small cystic mass on the right side of her uterus 4 years ago and did not pay attention to it. 1 year later, she was reexamined and grew to about 3 times the size with symptoms such as right lower abdominal pain and intermenstrual bleeding, so she came to our hospital.

I. Initial consultation
The patient had a small cystic mass on the right side of the uterus with a size of 1.9×1.6×1.8 cm, which was not treated. 1 year ago, the gynecological ultrasound of the external hospital suggested a cystic mass on the right side of the uterus with a size of about 6.1×5.7×6.2 cm, and the patient was recommended to have surgery. In the past two months, she felt pain in her right lower abdomen, which was vague, tolerable, paroxysmal, aggravated by exertion and relieved by rest, and was accompanied by intermenstrual bleeding, so she came to our hospital for consultation. Ultrasound examination of the pelvis showed that a 6.8×5.8×7.3 cm mass with clear border, oval shape, intact envelope, smooth wall and fluid dark area was detected in the upper right anterior part of the uterus; a 3.4×1.5 cm ovarian sound image was detected immediately behind the mass; a 2.1×1.9×2.0 cm mass was detected immediately behind the lateral border of the left ovary. The initial diagnosis was ovarian cyst and surgery was recommended, so she was admitted to hospital.
II. Treatment history
After the patient was admitted to the hospital, she completed relevant examinations and was considered to have a pelvic mass of unknown nature and was recommended to undergo surgery. The operation went smoothly and the patient returned to the ward safely after surgery. The postoperative examination revealed a right ovarian coronary cyst and a left ovarian plasmacytic cystadenoma.
III. Treatment results
The patient had obvious abdominal pain before treatment, and ultrasound examination suggested cystic ovarian masses. One week later, the patient reported that her abdominal pain was relieved and on examination, the surgical incision healed well without infection and no other complications.
IV. Notes
I was very happy to see that the patient recovered well. Before the patient was discharged, I instructed the patient to pay attention to the following points.
1. pay more attention to rest, establish regular habits of work and rest, ensure enough sleep and avoid staying up late.
2, in order to promote the recovery of the disease, should strengthen the management of diet, choose food rich in high-quality protein, such as milk, eggs, fish and meat.
3. maintaining a positive and optimistic mood and avoiding stress, anxiety and depression can reduce the risk of disease recurrence.
4.Patients should have regular medical checkups after discharge from the hospital, which is recommended once a year, and seek medical attention promptly if there are uncomfortable symptoms.
V. Personal insight
Ovarian cysts, generally with no obvious symptoms, are mostly found during routine physical examinations. With the gradual increase of the cyst, abdominal pain, abdominal distension and pressure in the lower abdomen may occur. Once a pelvic mass is detected by ultrasound, a professional physician needs to be consulted to assess the nature of the mass and to develop an appropriate treatment plan. In this case, the patient was not given timely targeted treatment measures, which led to the continuous increase of the cyst and seriously affected the patient’s quality of life, but fortunately the patient recovered after active treatment.
