I. Initial consultation
Patient’s self-report: repeated redness, swelling, peeling and peeling around the lips, with irregular location, occurring at regular intervals, occasionally with painful symptoms, affecting the aesthetics, as well as affecting eating, for unknown reasons. After understanding, the patient likes to buy some cheap lipstick before the onset, and the peeling of lips will be aggravated after applying lipstick. Occasionally stay up late in the day, eat fruits, drink less water, like to drink carbonated drinks, like to eat spicy hotpot, spicy hotpot, spicy strips, etc.. Oral examination can be seen: the upper and lower lip red and skin intersection position, bilateral corners of the mouth area dry without secretions, the continuous appearance of white fine chaff-like scales, varying thickness, easy to peel off, some areas scales off and then new flakes, lip tissue loss of elasticity, the formation of folds and chaps. Some areas of the lower lip had limited lip redness, mucous membrane thickening, and hyperkeratosis. Combined with the patient’s medical history, the diagnosis of desquamative labyrinthitis was made.
(face-to-face consultation)
II. Treatment history
1.Medication: use compound chlorhexidine with gargle wet compress for sterilization and anti-inflammation, and fluorouracil cream for lip application to improve hyperkeratosis of the lips, with a follow-up visit in 7-10 days. Daily oral multivitamin tablets to increase trace elements in the body and promote regeneration of lip tissue.
2. Life conditioning: Inform patients to change their dietary habits, regularize three meal times a day, eat a light diet with green leafy vegetables and lean meats as the mainstay, and usually consume more fresh fruits such as kiwi and apples, or squeeze fruits into juice for drinking. Avoid eating too hot and overheated food, such as spicy hotpot, hot pot, etc., to avoid affecting the healing of the lip wound. Patients are advised not to use poor quality lipstick, and after local healing, they can use lip balm to apply locally. Avoid sun exposure, and pay attention to wearing sun hat and mask when playing outdoor sports.
III. Treatment effect
The patient returned to the clinic on the 7th. Before the treatment, the lips were flaky, the elasticity of the red lip skin was poor, and pain occurred occasionally when eating; after the treatment, the symptoms of flaky lips were relieved, the elasticity of the red lip skin was improved, and the pain was significantly relieved when eating. At the initial consultation, the patient was emotionally tense and mentally stressed, but by adjusting the work and rest time and diet, the mental stress became significantly less, the tension was relaxed and the sleep quality was improved. 1 month later, the patient was followed up by telephone and said that he was cured, no discomfort and the treatment effect was good.
IV. Notes
I am glad that the patient’s symptoms were relieved after treatment, and I told the patient to pay attention to choosing the right products for himself when choosing cosmetics, especially lipstick, and make sure to pay attention to the ingredients so as not to cause allergic reactions to the lips and become a bad irritant. Try to avoid the habit of biting lips on a daily basis and pay attention to oral hygiene to avoid aggravation of lip infection. Regular work and rest time, early to bed and early to rise, enhance physical exercise, do more aerobic exercise. In terms of diet, regular three meals a day, reasonable nutrition, drink more warm water, avoid spicy and stimulating food, avoid overly hot food, and it is recommended to stay away from fried food.
V. Personal insight
Young women like to wear makeup, but it is more important to choose the right formal cosmetics, and some women have a more sensitive constitution, so they should pay attention to protecting their skin. The onset of labyrinthitis is more common in women than in men, and it also has a recurring character, with obvious seasonality, generally starting in late spring, worsening in summer, and reducing or receding in autumn. In general, those with mild symptoms and no discomfort do not need special treatment and can heal themselves in 2-4 weeks, mainly by regulating their mood, eating lightly, developing good oral and lip cleaning habits and removing undesirable stimuli. However, if, as in this case, there has been pain, allergic reaction, or combined with bacterial fungal infection, local anti-inflammatory symptomatic treatment is needed. In daily life, pay attention to dietary hygiene, eat a light diet, avoid stimulating snacks and beverages, and avoid foods that can easily cause allergic reactions such as seafood, fish and shrimp, and stimulating condiments.
