Misconceptions of burn treatment and the correct treatment method

  Due to the gradual increase of patients from the surrounding areas, the outpatient and inpatient services of our burn department have continued to grow exponentially in recent years, ranking steadily at the highest level in Zaozhuang City and at the forefront of Shandong Province. In the process of treating a large number of patients referred to us, we found that many patients had their wounds infected or artificially deepened due to improper early treatment, which increased the patients’ pain, caused functional impairment and even disability, leaving lifelong regrets. Here is a brief introduction to the common misconceptions about the early treatment of burns: Misconception 1: the trauma is exposed quickly The trauma is dry and exposed, so that its crusting heals faster. Basic and clinical research at home and abroad has long confirmed that trauma drying and dehydration will aggravate progressive necrosis of trauma stasis zone tissues, increase trauma pain, damage residual attachments with regenerative capacity, lead to deepening of trauma and delay trauma healing. The current medical consensus is that only in wet conditions can host phagocytes function to promote inflammation regression and prevent infection. A wet environment also prevents the exposure and death of nerve endings, reduces wound pain, facilitates the dissolution of necrotic tissue and promotes the release of multiple growth factors, providing an ideal environment for wound healing, thus promoting wound healing.  Misconception two: smear on the trauma Burn, burns, many people apply toothpaste, soy sauce, alcohol, purple potion, red mercury, gentian violet smear on the wound, etc., not knowing that these are harmful to the wound instead. In the burn wounds coated with colored drugs will affect the doctor’s judgment of the depth of burns, the depth of burns is a very important basis for the doctor to develop a treatment plan. Furthermore, there are many folk “native methods” for treating burns or scalded wounds, such as applying badger oil, herbal ash, or even some Chinese medicinal preparations to the wound. From the point of view of modern medicine, these folk methods of treatment often cause infection and aggravate trauma damage, and are therefore undesirable. The correct application of drugs are: safflower oil, petroleum jelly, moist burn cream, etc. can effectively reduce trauma pain, but also has a certain role in protecting the trauma, the site can be applied in appropriate amounts on the trauma, and then go to a professional hospital.  Myth 3: You can’t use cold water after the injury Some people think that you can’t use cold water after the burn, otherwise you will get blisters. In fact, the blisters after the burns are only related to the cause and depth of the burns, and not to whether or not they are exposed to cold water.  The correct treatment is: a small area of burns or burns trauma with tap water rinse or soak rinse about 20 ~ 30 minutes. It can reduce the residual heat from the scald or burn site to continue to damage the skin tissue, which can relieve pain and reduce tissue edema and blister formation. In addition some burns are caused by chemicals, timely and effective rinsing can flush out toxic chemicals so that the chemicals are not absorbed and poisoned or invaded into deep tissues for continued damage. The temperature of the water should be as hot as the patient can tolerate, usually 15℃~20℃. Ice can be added in summer if the temperature is higher. Then the trauma is simply bandaged, protected and sent to a specialist hospital for further treatment.  Myth 4: Burns are only skin injuries. Many people think that burns are only superficial injuries and can be bandaged in a small clinic. In fact, burns are very serious trauma, larger area (adults burn area of 20%, infants and children burn area greater than 5% body surface area) of burns may soon lead to shock, not in time to give shock resuscitation, several hours heart, liver, brain, lungs, kidneys, gastrointestinal organs will be different degrees of damage, serious life-threatening. So serious burns should be immediately to a specialist hospital to prevent accidents.  Myth 5: Some kind of drug has special effects. Some kind of panacea or ancestral secret recipe can cure all burn wounds. In fact, the depth of burns caused by different burn causes, different heat source temperatures, different contact times, and different burn sites are definitely different. Different burn sites, different burn depths, different treatment periods, different burn causes have strict indications for medication, never blindly believe in a particular drug or a particular treatment method, experienced professional doctors will develop appropriate treatment plans according to different circumstances.  Myth 6: You can’t eat hairy food after injury The so-called hairy food refers to certain foods that can aggravate the disease or recurrence of old diseases, such as fish, shrimp, mutton, dog meat, leek, parsley, etc. In fact, hairy food is a folk saying, and in practice we have not found any actual cases of aggravation or relapse of old diseases after eating hairy food. On the contrary, because these foods contain a variety of vitamins and proteins, but for the healing of burn wounds have a lot of help. Therefore, as long as there is no special reaction to these foods in general, you can eat more after the burn to facilitate the healing of the wound.
