Rehab Academy | Medical gymnastics for hemiplegia

Principle
To induce and mobilize the contractile response of the muscles on the affected side by enhancing the active or resistive activity of the healthy limb to promote the generation of joint responses and co-movement.
The reflexive inhibition of postural and control key points in the neurodevelopmental facilitation technique is used to inhibit the flexor spasticity pattern of the upper limb and the extensor spasticity pattern of the lower limb on the hemiplegic side.
Stimulation of the affected limb using cutaneous sensory facilitation techniques to enhance sensory input by increasing the patient’s attention to the affected limb. Details
Hair combing with healthy hands
Method: Turn the head to the affected side and use the able-bodied hand to comb the hair from the forehead of the able-bodied side to the back of the head and neck, requiring the fingers to press firmly on the scalp and slowly push backward, repeating 20 times.
Patting with the healthy hand
Method: Place the affected arm in front of the chest and use the palm of the healthy hand to pat from the affected shoulder along the lateral side of the upper limb to the hand, carry out 20 times round trip.
Squeeze the affected hand
Method: Turn the affected arm to the chest with the healthy hand, use the thumb and index finger of the healthy hand to squeeze each finger along the affected side from the distal end to the proximal end, and press firmly at the root of the proximal end of the finger for 20 seconds, and repeat each finger 5 times.
Group finger upraise
Method: Cross the affected hand with the healthy hand in front of the chest, press the thumb of the affected hand on the thumb of the healthy hand, then the healthy hand drives the affected hand to lift forward or up over the head until both elbow joints are completely straight, hold for 10 seconds and then recover, repeat 20 times.
Circumferential face washing
Method: Grasp the affected hand with the healthy hand to make it stretch, and then make an imitation of washing the face in the forward and reverse direction with the healthy hand, repeat 10 times.
Half bridge exercise
Method: Stretch the two upper limbs on the side of the body, bend the two lower limbs to the hip and knee position, fix the affected lower limb or cross the affected leg on the healthy knee, lift the hip off the bed as far as possible, hold for 10 seconds, repeat 5~10 times, no breath-holding action.
Resistance leg clamping
Method: Bend both lower limbs at the hip and knee, support both feet on the bed, have someone else fix the affected leg, then rotate the healthy leg inward to the affected leg, while having someone else apply some resistance on the inside of the healthy knee to enhance the power of completing the resistance pinch, repeat 20 times.
Leg crossover swing
Method: The affected leg is supported by passive hip flexion and knee flexion, the foot is fixed by others, the healthy leg is crossed over the affected knee, the hip is swung to the left and right driven by the healthy leg, the healthy leg is required to play a fixed role on the affected leg during the activity, repeat 20 times.
Straight leg raise
Method: Raise the lower limb of the healthy side in a straight position by 30 degrees, hold for 10 seconds, or hold the healthy leg over the affected leg to do straight leg raise, repeat 5 times.
Touching hands and feet
Method: Use the hand on the healthy side to touch the back of the foot of the healthy leg, repeat 10 times.
Knock on the knee with the healthy foot
Method: Tap the affected knee with the heel of the healthy foot, from below the knee along the front and outside of the calf from the top down to the outside of the foot and back 10 times.
Precautions
Strengthen the protection of the joints of the affected limb to prevent joint injury
Prevent breath-holding during all rehabilitation exercises
Ask the patient to pay more attention to the affected limb
Try to mobilize the subjective initiative and actively cooperate with the therapist
