1. Indications for adjuvant chemotherapy after breast cancer surgery (1) Tumor >2 cm. (2) Positive lymph nodes. (3) Hormone receptor negative. (4) HER-2 positive (there is no clear evidence to recommend adjuvant chemotherapy for patients below T1a). (5) Histological grading of grade 3. The adjuvant chemotherapy regimen should be developed taking into account the clinicopathological characteristics of the tumor, patient-side factors and the patient’s wishes, as well as the possible benefits of chemotherapy and the adverse effects brought about by it. Immunohistochemical tests should routinely include ER, PR, HER-2 and Ki-67. Contraindications (1) Pregnancy: Chemotherapy should be carefully selected for patients in early and mid pregnancy. (2) Elderly and frail patients with severe visceral organic lesions.
