On September 9-10, two consecutive cases of 3D thoracoscopic cardiac surgery were completed in our cardiac surgery department, both of which were successful. After being checked by the Ministry of Education’s Science and Technology Research Station, no literature reports on the application of 3D thoracoscopic techniques for mitral valve replacement and atrial septal defect repair were found in published Chinese and foreign literature.
In Patient 1, a 62-year-old patient with mitral valve lesion, the surgeon made three keyhole-sized incisions in the chest to complete the entire cardiac procedure. The patient’s femoral artery and vein were cannulated at the base of the thigh to establish extracorporeal circulation, and then the view of the cardiac surgery was transmitted to a television screen through a three-dimensional thoracoscope inserted into the right side of the chest cavity. The operation is performed by inserting a keyhole incision into the chest cavity with special surgical instruments, stopping the heart, incising the right atrium and septum, removing the diseased mitral valve, replacing an artificial biological heart valve, suturing the heart incision, venting the heart cavity, opening the aorta, restoring the blood supply to the heart, and resuming the heart. The operation lasted 2.5 hours, and the patient was taken off the ventilator assistance 2 hours after the operation and was out of bed on the third day with good results. Patient 2, female, 28 years old, suffering from congenital heart disease atrial septal defect, the doctor completed the atrial septal defect repair under 3D thoracoscopy without stopping, the operation lasted 2 hours, the operation went smoothly and the patient could get out of bed and move around the next day.
