Treatment of chronic colitis

  Chronic colitis is a common clinical disease, the pathogenesis of which is related to a variety of factors, is a chronic non-specific inflammation of the mucosal layer of the colon and rectum. Clinical manifestations of diarrhea, abdominal pain, abdominal distension or urgency, stools with mucus or pus and blood, often accompanied by fatigue, poor performance, unclean bowel movements, anal swelling, mood irritability and other symptoms. It brings a lot of inconvenience to the normal work and life of patients. Modern medicine for this disease is not yet a special treatment drugs. The Chinese medicine believes that chronic colitis is a Chinese medicine “diarrhea”, the location of the disease in the intestines, organs in the liver, spleen, stomach and kidney.  I, the etiology of the pathogenesis in the same external environmental conditions, affected by the same pathogenic factors, some people develop, others do not develop, look into the reasons: “Jing Yue Quan Shu”: “diarrhea and drainage of the root, all by the spleen and stomach. The spleen is the master of transportation and transformation, and is the “source of biochemistry of qi and blood” and the “source of the latter day”. Due to congenital or due to poor diet, fatigue, and long days leading to damage to the spleen and stomach, the spleen is not healthy, then the qi and blood are not enough to moisten the whole body, phlegm and drinks are generated internally, or dampness and evil turn into heat and cold over time; stopping in the intestines, the large intestine conduction is out of order, and the qi and blood and the intestinal dregs are knotted together, damaging the intestinal complex. Long-term diarrhea does not heal, and the kidneys are affected over time, resulting in spleen and kidney Yang deficiency. In addition, emotional injury, liver loss of drainage, liver qi transverse to offend the spleen and earth, liver and spleen disharmony, liver depression and spleen deficiency, resulting in spleen and stomach damage.  In conclusion, the disease is a mixture of deficiency and reality, with the original deficiency and the symptoms being real. Weakness of the spleen and stomach is the main cause; damp-heat, phlegm-dampness and blood stasis are the symptoms. On the other hand, the deficiency of positive energy is easily injured by external sensation, diet and emotion, which makes it difficult to heal and recurrent.  Any disease is a process of change and development, and there is a key link at each stage. We should combine the identification of evidence and disease according to the characteristics of the disease, “treat the symptoms if it is urgent, and treat the root cause if it is slow”, and give equal importance to supporting the righteous and eliminating the evil.  1.Acute attack: diarrhea, abdominal pain, abdominal distension, mucus-purulent-blood stool more than 10 times a day, shortness of breath, fatigue, poor appetite and emaciation, red tongue, yellow and thick greasy coating, pulse count. Routine stool examination: a large number of red and white blood cells (e.g., 20-30 red blood cells, full field of white blood cells). Most of the cases at this stage are due to the evil of dampness and heat, stagnation in the intestine, and the blood and qi, blood defeat and flesh decay. Treatment is to clear heat and dampness, cool the blood and stop bleeding.  2. Chronic phase: This phase is relative to the acute attack phase, there may still be mucus-purulent-blood stool, but the stool routine + occult blood shows: 0-8 red blood cells and 1-5 white blood cells, which is significantly less than before. (1) Spleen deficiency and damp-heat type: see diarrhea and abdominal pain, wrist and abdomen fullness, stool several times a day, mucus and pus and blood, anal burning, feeling of unclean bowel movements, weakness and poor circulation, thirst and desire to drink, short and red urine, red tongue, yellow and thick greasy coating, string pulse. Treatment is to clear heat and dampness, benefit Qi and strengthen the spleen.  (2) Spleen qi weakness type: symptoms include abdominal pain, loose stools, several times a day with mucus or indigestible diet, symptoms are aggravated after eating cold and greasy food, with withered face, light red tongue, thin white fur, and weak pulse. Treatment is to benefit the qi, strengthen the spleen, and relieve dampness.  (3) Spleen and kidney yang deficiency type: i.e., generalized cold symptoms, diarrhea and abdominal pain when cold is encountered.  (4) Liver and spleen disharmony: symptoms include abdominal pain, bloating, diarrhea or constipation, with mucus in the stool once a day or several times a day, pain decreases after the stool, especially aggravated by emotional disorders, with a light red tongue, thin white fur, and a weak pulse. Treatment is to dredge the liver and strengthen the spleen.  (5) Retention enema treatment method This method is a combination of local and overall treatment. The drug reaches the disease directly and takes effect, avoiding the influence of the digestive tract on it, maintaining the performance of the drug, quickly and fully exerting the therapeutic effect, and its curative effect is sure. The use of dialectical medicine: Huang Lian, Lian Qiao, Licorice, etc., together with oral Chinese medicine to play a half-hearted effect. For patients with positive fecal occult blood, 3g of Panax ginseng powder or 2g of Qing Dai can be added to the decocted enema to activate the blood and stop bleeding and clear heat; if the local ulcer is heavy, 1g of tin-like powder (watermelon cream, borax, Qing Dai, ice chips, pearl, niuhuang, etc.) can be added to detoxify the swelling and relieve pain. Fifteen days is a course of treatment, after which it is discontinued for one week, followed by the next course of treatment.  However, this disease originates from the weakness of the spleen and stomach, and is prone to recurrence, so prevention is important. We should pay attention to keeping warm, moderate diet, and strengthen exercise. In addition, the increasing pressure of work life in today’s society is one of the important reasons why the incidence of the disease is on the rise, so it is important to maintain a good state of mind and a relaxed mood.
