What is temporomandibular joint disorder?

       Temporomandibular joint disorder (TMJ) syndrome is the internationally accepted name for TMJ disorder (TMD) in recent years. It is not a single disease, but a group of diseases with the same or similar clinical symptoms whose etiology is not fully understood. It generally has three types of symptoms: myalgia of the temporomandibular joint area and/or masticatory muscles; abnormal jaw movements and associated dysfunction; and joint popping, crushing sounds and murmurs. The TMJ or masticatory muscles may be involved alone, or both. Most of the disease is functional disorder in nature, but it may also involve structural disorders of the joint or even organic damage.  The causes of TMJ disorders have not yet been fully elucidated. Most scholars have suggested factors related to the pathogenesis of the disease based on experimental and clinical studies, and all of them believe that the disease is multifactorial in nature, and are generally considered to be related to the following factors: 1. psychosocial factors, patients often have emotional anxiety, irritability, mental tension, agitation and insomnia; 2. Immune factors; 4. Overloading of joints; 5. Anatomical factors, such as obvious weakening of the corresponding joints and muscle ligaments, and reduced weight-bearing capacity of the joints; 6. Other factors, such as cold stimulation of the joint area, poor posture causing muscle dysfunction and affecting the normal position of the mandible and condyle, etc.  Clinical manifestations The development of TMJ disorder can have three stages: functional disorder stage, structural disorder stage, and organic joint destruction stage. These three stages generally show the early, middle and late stages of the disease. Although the disease generally has a long duration, which can be years or decades, and is often recurrent, it is self-limiting and generally has a good prognosis if joint ankylosis does not occur. The clinical manifestations have the following three main features: 1. abnormal jaw movements including abnormal opening (too large or too small); abnormal opening pattern (skewed or distorted); and joint strangulation in opening and closing movements.  2. Pain Mainly manifested as pain in the joint area or peri-articular muscle groups during opening and chewing movements. There is usually no spontaneous pain. In some patients with prolonged disease, there is often sinking, soreness and swelling in the joint area, easy fatigue of the masticatory muscles, as well as chronic pain and abnormal sensation in the cheek, temporal area and occipital area.  3, popping and murmurs Normal joints have no obvious popping and murmurs during jaw movement. The common abnormal sounds in this disease are: (1) popping sound, i.e. “click, click” sound in the opening movement; (2) breaking sound, i.e. “click, click” breaking sound in the opening movement; (3) rubbing sound, i.e. continuous rubbing sound in the opening movement. (3) rubbing sound, that is, there is a continuous rubbing sound like rubbing cellophane in the opening movement.  The disease can be accompanied by various ear diseases, various eye diseases, as well as swallowing difficulties, speech difficulties, chronic general fatigue, etc.  Prevention and control principles 1. Conservative treatment is the main focus, using a combination of symptomatic treatment and elimination or reduction of curative factors.  2.Improve the general condition and mental status of the patient, including active psychological support treatment.  3.Patients should be educated on medical knowledge so that they can treat themselves, protect their joints and change their bad life behaviors.  4.The treatment procedure should be reversible conservative treatment first, then irreversible conservative treatment, and finally arthroscopic surgery and various surgical treatments.  V.
